刃针治疗面神经炎恢复期临床观察及对肌电图的影响

Clinical Observation of Blade Needle for Convalescence of Facial Neuritis and Its Influence on EMG

目的:探讨刃针治疗面神经炎恢复期的临床疗效及对肌电图的影响。方法:将70例患者按随机数字表法分为观察组和对照组,每组35例。两组均予VitB_1片10 mg/次,甲钴胺分散片0.5 mg/次,均3次/天口服,在此基础上,观察组予刃针治疗,对照组予常规针刺治疗。两组治疗时间均为30天。观察治疗前后多伦多(Sunnybrook)面神经评分系统评分及肌电图,并评价临床疗效,随访1年观察并发症情况。结果:观察组的总有效率为91.4%,高于对照组的80%(P<0.05);治疗后两组Sunnybrook评分、R1潜伏期较治疗前上升(P<0.05),CMAP波幅较治疗前上升,但差异无统计学意义(P>0.05);组间治疗后比较,观察组对Sunnybrook评分、CMAP波幅及R1潜伏期的改善优于对照组(P<0.05),观察组并发症发生率更低,但与对照组比较差异无统计学意义(P>0.05)。结论:刃针治疗面神经炎恢复期疗效确切,能促进神经功能修复,优于常规针刺,且并发症更少。

Objective： To explore the curative effect of blade needle for treating facial neuritis during convalescence and its influence on electromyogram（ EMG）. Methods： 70 cases were randomly divided into the observation group and the control group,with 35 cases in each group. Both groups were given Vitamin B1（ 10 mg） and Methylcobalamin（ 0. 5 mg）,three times a day,based on which,the control group was also given routine acupuncture,whereas the observation group was also given blade needle. The treatment durations in both groups were 30 days. The score of Sunnybrook facial nerve scoring system and the gram of EMG were recorded and compared before and after treatment to evaluate the curative effect. The occurrence of complications was observed during the one-year follow-up. Results： The total effective rate of the observation group was 91. 4%,which is significantly higher than 80% of the control group（ P〈 0. 05）; after the therapy,the Sunnybrook score and R1 latency were increased more than those of pre-treatment in the same group（ P〈 0. 05）,the CMAP amplitude was higher than that of pre-treatment in the same group,but the difference was not statistically significant（ P〈 0. 05）; after treatment,the observation group was improved better than the control group,in terms of Sunnybrook score,CMAP amplitude and R1 latency（ P〈 0. 05）; the complications in the observation group was less,but there was no significant difference between the two groups（ P〈 0. 05）. Conclusion： Blade needle has obvious therapeutic effect for treating facial neuritis during convalescence with less complications,and it can promote the recovery of nerve function,which is better than the routine acupuncture.
