噻托溴铵与普米克令舒及万托林在急性肺气肿患者中的疗效观察 被引量:3

摘要 目的观察噻托溴铵、吸入用布地奈德混悬液(商品名:普米克令舒)及吸入用硫酸沙丁胺醇溶液(商品名:万托林)在急性肺气肿患者中的临床治疗效果。方法 80例急性肺气肿患者,随机分为对照组和观察组,每组40例。对照组采用生理盐水、普米克令舒和万托林雾化治疗,观察组在对照组的基础上联合噻托溴铵治疗,比较两组临床疗效。结果观察组患者总有效率为95%,显著高于对照组的80%(P<0.05)。观察组患者治疗后的第一秒用力呼气量占预计值百分比(FEV1%)及第一秒呼出气量与用力肺活量之比(FEV1/FVC)值明显优于对照组(P<0.05)。观察组二氧化碳分压(PaCO_2)、氧分压(PaO_2)、动脉血氧饱和度(Sa O_2)、pH值优于对照组(P<0.05)。结论急性肺气肿患者采用噻托溴铵、普米克令舒和万托林联合用药治疗效果理想,安全性高,值得推广应用。 Objective To observe the clinical therapeutic effect of tiotropium bromide and budesonide suspension for inhalation (Pulmicor respules) and salbutamol sulfate solution for inhalation (Ventolin) in patients with acute emphysema. Methods A total of 80 patients with acute emphysema were randomly divided into control group and observation group, with 40 cases in each group. The control group received physiological saline, Pulmicor respules and Ventolin for atomization treatment, and the observation group received tiotropium bromide on the basis of the control group. Clinical effect was compared in two groups. Results The observation group had significantly higher total effective rate as 95% than 80% in the control group (P〈O.05). The observation group had obviously better forced expiratory volume in 1 second percentage of expected value (FEVI%) and forced expiratory volume in 1 second percentage of forced vital capacity (FEV1/FVC) than the control group (P〈O.05). The observation group had better partial pressure of carbon dioxide (PaCO2), oxygen partial pressure (PaO2), arterial oxygen saturation (SaO2), and pH value than the control group (P〈0.05). Conclusion Tiotropium bromide and Pulmicor respules and Ventolin combination therapy are ideal and safe for acute emphysema patients, and it is worthy of promotion and application.
