清得佳凝胶联合全程护理干预对头颈部肿瘤病人放射性皮肤损害的效果

CLINICAL EFFECT OF INTRASITE GEL COMBINED WITH WHOLE-COURSE NURSING INTERVENTION IN TREATMENT OF RADIATION-INDUCED SKIN LESIONS IN PATIENTS WITH HEAD AND NECK CANCER

目的探讨清得佳凝胶联合全程护理干预对头颈部肿瘤病人放射性皮肤损害的效果。方法选取首次接受放射治疗头颈部肿瘤病人45例作为对照组,51例作为实验组。对照组行皮肤常规护理;实验组实施全程护理干预,并在放疗皮肤野涂抹清得佳凝胶。结果实验组放疗依从性达96.08%,对照组放疗依从性为75.56%,两组比较差异有显著性(χ2=8.600,P<0.05)。实验组发生Ⅰ级放射性皮肤损害的放射剂量明显高于对照组,差异有显著性(t=2.221~2.688,P<0.05);实验组病人放射性皮炎、放射性疼痛及皮肤瘙痒的发生率明显低于对照组,差异有统计学意义(Z=2.169~3.250,P<0.05)。结论头颈部肿瘤病人实施清得佳凝胶联合全程护理干预,能有效减轻皮肤损伤、疼痛及瘙痒等症状,值得临床推广使用。

Objective To investigate the clinical effect of Intrasite Gel combined with whole-course nursing intervention in the treatment of radiation-induced skin lesions in patients with head and neck cancer. Methods A total of 96 patients with head and neck cancer who received radiotherapy for the first time were enrolled and divided into experimental group with 51 patients and control group with 45 patients.The patients in the control group were given routine skin nursing,and those in the experimental group were given whole-course nursing intervention combined with external application of Intrasite Gel on the skin exposed to radiotherapy. Results The experimental group had a significantly higher rate of radiotherapy compliance than the control group（96.08% vs 75.56%,χ2=8.600,P〈0.05）.The experimental group had a significantly higher radiation dose for gradeⅠradiationinduced skin lesions than the control group（t=2.221-2.688,P〈0.05）.The experimental group had a significantly higher proportion of patients with grade≥2radiation dermatitis,radiating pain,or pruritus than the control group（Z=2.169-3.250,P〈0.05）. Conclusion In patients with head and neck cancer,Intrasite Gel combined with whole-course nursing intervention can effectively alleviate skin lesions,pain,and pruritus and holds promise for clinical application.
