摘要
日间手术在我国已推进10多年,目前上海等地已积累丰富经验,形成了成熟的管理模式,但日间手术流程仍需改造、优化。文章基于流程再造理论,通过文献研究、理论分析与数据处理、深度访谈等研究方法,总结归纳了IAAS模式、四川华西模式、上海申康模式、北京同仁模式的优缺点,选取某三甲医院日间手术管理模式为例,研究该院门诊、术前检查、麻醉评估、手术预约、手术、出院、术后支持指导等7个环节,综合评价管理现状和效果,重点探讨门诊、术前检查、麻醉评估、日间手术中心等4个环节的优化,充分听取医师和患者建议,综合基础条件、政策、信息、技术等因素影响,提出理想的日间手术管理流程,以供各医院参考。
Ambulatory surgery has been advanced for more than 10 years in China, experiences of which have been summa- rized to form a mature mode of management. But the ambulatory surgery process still needs to be optimized and reformed. Based on the theory of process reengineering, the advantages and disadvantages of several management modes were summarized by litera- ture studies, theoretical analysis and data analysis, depth interviews and other research methods. Then a grade three and first - class hospital was chosen to study its seven processes, which focused on four processes of outpatient clinic, preoperative examina- tion, anesthesia evaluation, and ambulatory surgery center. Furthermore, combined with the advice from physicians and patients, and factors of basic condition, policy, information and technology, the management process of ambulatory surgery was put forward.
