伴上颌窦囊肿的上颌窦外提升术后牙种植1例

A case report of maxillary sinus augmentation with maxillary sinus cyst for dental implant

目的通过1例伴上颌窦囊肿的上颌窦外提升术10个月后行牙种植术的病例报告,总结此类患者的治疗经验。方法上颌窦外提升术前锥形束CT(CBCT)发现缺牙区上颌窦有椭圆形边界清楚的低密度影,术者进行上颌窦外提升术,10个月后发现囊肿致术区颊侧骨缺损,且囊壁暴露,术者遂抽出囊液,完整摘除囊肿,0.9%氯化钠溶液加庆大霉素彻底冲洗上颌窦,骨缺损处放置富血小板纤维蛋白(PRF)膜。结果种植体植入6个月后CBCT示骨量稳定,骨结合良好,囊肿无复发。结论在伴有囊肿的上颌窦外提升术后种植的病例,若在牙种植术时发现囊肿暴露,应及时摘除囊肿,以防囊肿影响种植体骨结合。

Objective Reporting a case of maxillary sinus augmentation implant with maxillarysinus cyst,to summarize the treatment experiences for the patients. Methods Before maxillary sinusaugmentation,CBCT showed the edentulous maxillary sinus with low density shadow,diagnosed asmaxillary sinus cyst. The cyst was removed completely 10 months after the augmentation due to theobservation of buccal bone defect and exploration of the cyst wall. The saline and gentamycin were used toirrigate the maxillary sinus and the platelet rich fibrin（PRF）membrane was placed on the bone defectsite. Result Six months after implantation,CBCT showed stable osseointegration and no recurrence ofcyst. Conclusions In such cases as our reported,the cyst should be removed promptly if the cyst isexposed at the time of implantation,in order to prevent the cyst from affecting the osseointegration of theimplant.
