老年胃癌患者营养状况调查及其与不同临床因素的相关性

Survey on Nutritional Status of Elderly Patients with Gastric Cancer and the Correlation Analysis on Different Clinical Factors

目的调查老年胃癌患者营养状况,分析我院老年胃癌患者营养状况与不同临床因素的相关性。方法横断面调查我院2013年9月至2016年4月收治的133例年龄≥65岁住院老年胃癌患者。首先应用患者主观整体营养评估量表(PG-SGA)进行调查,并对患者进行相关的实验室检查,探讨老年胃癌患者营养状况不良的相关临床因素。结果 133例住院老年胃癌患者中,出现营养状况不良者有130例,占97.73%,其中轻度营养不良22例、中度营养不良51例、重度营养不良57例。营养状况与临床因素分析结果示:无或轻度营养不良患者的前白蛋白为(223.76±54.41)mg·L^(-1),高于中至重度营养不良患者的(182.54±64.29)mg·L^(-1),差异有统计学意义(P<0.05);无或轻度营养不良患者的总住院时间为(10.56±6.53)d,低于中至重度营养不良患者的(15.32±9.66)d,差异有统计学意义(P<0.05)。结论临床上应注重老年胃癌患者的营养状况,尽早筛查评估,同时做好前白蛋白水平的监测并予以及时干预,有助于缩短住院时间,改善预后。

Objective To investigate the nutritional status of elderly patients with gastric cancer, and to analyze the correlation between the nutritional status of elderly patients with gastric cancer in our hospital and different clini- cal factors. Methods The cross-sectional survey was implemented on 133 elderly patients with gastric cancer at the age of no less than 65 hospitalized from September 2013 to April 2016. The patient-generated subjective global as- sessment （PG-SGA） was used for survey, and the related laboratory tests were implemented, and the clinical fac- tors associated with poor nutritional status of elderly patients with gastric cancer were discussed. Results Among 133 elderly patients with gastric cancer, there was 130 patients in poor nutritional status, accounting for 97.73%, among them, 22 patients for mild naalnutrition, 51 patients for moderate malnutrition and 57 patients for severe mal- nutrition. Nutritional status and clinical factors analysis： the prealbunain for patients without or with mild malnutri- tion was （223.76 ±54.41 ） mg · L-1, and was （182.54 ±64.29） mg·L-1 for patients with moderate and severe malnutrition （P 〈 0.05） ; the total hospitalization days for patients without or with naild malnutrition was （10.56 ± 6.53） days, and was （15.32 ±9.66） days for patients with moderate and severe malnutrition （P 〈 0.05 ）. Con-clusion The nutritional status of elderly patients with gastric cancer should be stressed, screening assessment shall be given as soon as possible, and at the same time prealbumin level monitoring and timely intervention shall be pro- vided, which can help to shorten the hospital stay and improve the prognosis.
