以全科医生为主体的糖尿病社区管理模式的应用价值分析

Application Value Analysis of the Diabetes Community Management Mode Based on General Practitioner as the Main Body

目的研究以全科医生为主题的糖尿病社区管理模式对糖尿病患者的应用价值。方法纳入2015年6月—2016年6月在深圳市罗湖区东门街道社康中心管理的298例糖尿病患者作为研究对象,随机分为两组。对照组患者采用常规糖尿病社区管理模式进行管理;观察组患者采用以全科医生为主体的糖尿病社区管理模式进行管理。1年后比较两组患者管理前后各项生化指标情况、依从性以及糖尿病患者生活方式和认知水平情况。结果管理前两组患者各项生化指标水平差异无统计学意义(P>0.05);管理后观察组各项生化指标水平优于对照组,差异有统计学意义(P<0.05)。观察组依从率高达85.2%,高于对照组的66.4%,差异有统计学意义(P<0.001)。观察组戒烟酒、定期运动、知晓糖尿病危害以及知晓并发症比例均高于对照组,差异有统计学意义(P<0.001)。结论以全科医生为主体的糖尿病社区管理模式对糖尿病患者的各指标水平控制较好,有效提高患者对疾病的认知,控制病情。

Objective To study the application value of the diabetes community management model based on the general practitioner as the main body.Methods 298 cases of diabetes patients received community management in community health service center of our community from June 2015 to June 2016 were studied.All cases were randomly divided into two groups.In the control group,patients were treated with routine diabetes community management model,and the observation group were treated with the general practitioner as the main body of the diabetes community management model.After 1 years,two groups of patients were compared before and after the management of the biochemical indicators,compliance,and the way of life of diabetic patients and cognitive level.Results Before management,there were no differences in the biochemical indexes of the two groups of patients（P〉0.05）;After administration,the biochemical indexes in the observation group were better than those in the control group,the difference was statistically significant（P〈0.05）.The compliance rate of the observation group was 85.2%,higher than that of the control group of 66.4%,the difference was statistically significant（P〈0.001）.Observation group of smoking cessation,regular exercise,awareness of the risk of diabetes and awareness of the proportion of complications were higher than the control group,the difference was statistically significant（P〈0.001）.Conclusion General practitioner as the main body of the diabetes community management model for the control of the indicators of diabetic patients is better,effectively improve the patient＇s awareness of the disease,control the disease.
