综合护理模式在48例颈椎骨折脱位合并脊髓损伤病人的应用

The application of comprehensive nursing mode in 48 patients with cervical spine fracture-dislocation complicated with spinal cord injury

目的:分析颈椎骨折脱位合并脊髓损伤的护理特点以及针对临床并发症和神经功能损伤的处理要点。方法:选取颈椎骨折脱位合并脊髓损伤病人48例,采取随机数字表法分为观察组与对照组,各24例,对照组病人采用常规普通护理模式,观察组病人采用综合护理模式,比较2组病人术后并发症发生情况及病人症状自评量表评分。结果:观察组病人压疮发生率显著低于对照组(P<0.01)。观察组病人人际关系敏感、强迫症状、躯体化、精神病性、偏执、抑郁、焦虑、敌对、恐惧和其他评分均显著低于对照组(P<0.01)。结论:综合护理模式相较于普通护理模式在颈椎骨折脱位合并脊髓损伤病人的术后护理中具有明显优势,病人并发症发生率低,症状自评量表评分情况良好,在一定程度上提高了病人的生活质量。

Objective： To analyze the nursing characteristics of 48 patients with cervical spine fracture-dislocation complicated with spinal cord injury,and provide the treatment measures in clinical complications and dysneuria. Methods： Forty-eight patients with cervical spine fracture-dislocation complicated with spinal cord injury were divided into the observation group and control group according to the random number table method（ 24 cases each group）. The observation group and control group were nursed with comprehensive nursing mode and conventional nursing mode,respectively. The incidence of adverse reactions and SCL-90 score between two groups were compared. Results： The incidence rate of pressure ulcer in observation group was significantly lower than that in control group（ P〈0.01）. The scores of sensitive interpersonal relationship,obsessional symptom,somatization,psychotic symptom,paranoia,depression,anxiety,hostility,fear and other items in observation group were significantly lower than those in control group（ P〈0.01）.Conclusions： Compared with the conventional nursing mode,the comprehensive nursing mode has significant advantages,the incidence rate of adverse reaction is low,the SCL-90 scoring condition is good,which can improve the quality of life in patients with cervical spine fracture-dislocation complicated with spinal cord injury.
