2903例健康体检人群胃镜检查结果分析

Analysis of gastroscopic results in 2903 people who received health examinations

目的探讨健康体检人群上消化道疾病谱。方法回顾性分析2014年7月~2015年6月在本院国宾部进行健康体检的2 903例人群胃镜检查结果,并选取同期1 475例特需消化科门诊患者进行对照。结果体检组常见的疾病为慢性胃炎(阳性率86.50%)、反流性食管炎(8.20%)、胃息肉(5.24%)、十二指肠球炎(3.17%)、消化性溃疡(1.89%)、食管裂孔疝(1.41%);门诊组为慢性胃炎(93.22%)、反流性食管炎(6.92%)、胃息肉(6.58%)、十二指肠球炎(3.46%)、食管裂孔疝(3.32%)、消化性溃疡(3.14%)。体检组未发现食管癌,胃癌阳性率0.07%;门诊组食管癌阳性率0.81%,胃癌阳性率0.68%(P<0.01)。结论健康体检人群上消化道恶性肿瘤发病率较低,绝大部分为良性疾病,广泛开展胃镜筛查可行性较差,仅适于少部分有需求的高端体检人群和部分特殊人群。

Objective To explore the spectrum of upper digestive tract disease in people receiving health examinations. Methods Gastroscopic results were retrospectively analysed in 2903 people who received health examinations from July 2014 to June 2015 and were compared with 1475 outpatients from the digestive department in a special consultation clinic at the same time. Results The common diseases in the health examination group were chronic gastritis （positive rate 86.50%）,reflux oesophagitis （8.20%）,gastric polyp （5.24%）,duodenitis （3.17%）,peptic ulcer （1.89%）and oesophageal hiatal hernia （1.41%）. The common diseases in the clinic group were chronic gastritis （positive rate 93.22%）,reflux oesophagitis （6.92%）,gastric polyp （6.58%）, duodenitis （3.46%）,oesophageal hiatal hernia （3.32%）and peptic ulcer （3.14%）. No oesophageal carcinoma was found in health examination group, and the positive rate of gastric carcinoma was 0.07%. The positive rates of oesophageal and gastric carcinoma were 0.81%（P=0.000） and 0.68%（P=0.001） in the clinic group, respectively. Conclusion The incidence of malignant tumours in the upper gastrointestinal tract in people who received health examinations is low, and the disease is mostly benign. The feasibility of widespread endoscopy screening in health examinations is unfavourable and is only suitable for particular populations.
