血必净注射液治疗坠积性肺炎疗效的系统评价

Systematic evaluation of clinical effects of Xuebijing Injection for patients with hypostatic pneumonia

目的:系统评价常规治疗联合血必净注射液治疗坠积性肺炎的临床疗效。方法:计算机检索Pub Med、Science Direct、Springer Link、EMbase、CNKI、VIP以及万方数据库,检索年限均从建库到2017年2月,纳入血必净注射液治疗坠积性肺炎的随机对照试验(RCTs),评价文献的质量并提取有效资料,采用Rev Man 5.3软件进行Meta分析。结果:共纳入8个研究,558例患者。常规治疗联合血必净注射液治疗坠积性肺炎在总有效率[RR=1.20,95%CI(1.12,1.28),P<0.000 01]及体征指标体温[RR=-1.89,95%CI(-2.57,-1.20),P<0.000 01][RR=-0.31,95%CI(-0.55,-0.07),P<0.01]、咳嗽[RR=-3.28,95%CI(-3.92,-2.63),P<0.000 01]、血象[RR=-2.25,95%CI(-3.50,-0.99),P=0.000 5]、肺片[RR=-1.79,95%CI(-2.39,-1.19),P<0.000 01]和白细胞(WBC)变化[RR=-1.58,95%CI(-2.83,-0.33),P=0.01]方面有统计学差异,而在呼吸变化方面无统计学差异。结论:常规治疗联合血必净注射液治疗坠积性肺炎的临床疗效优于常规治疗组,但受纳入研究质量和数量所限,上述结论尚需更多大样本、多中心的随机对照试验加以验证。

Objective：To systematically review the efficacy of Xuebijing Injection combined with routine therapy versus routine therapy alone in treating hypostatic pneumonia.Methods：Pub Med,Science Direct,Springer Link,EMbase,CNKI,VIP and WanFang Data were searched to retrieve the randomized controlled trials（RCTs） on Xuebijing Injection for hypostatic pneumonia from the date of establishment until February 2017.Two reviewers independently evaluated the quality of literature and extracted data.Rev Man 5.3 software was used for Meta-analysis.Results：Eight RCTs（558 patients） were included.The results of Meta-analyses showed that routine therapy plus Xuebijing Injection group had highter total effective rates[RR=1.20,95% CI（1.12,1.28）,P〈0.000 01],and they had better improvement in body temperature[RR=-1.89,95% CI（-2.57,-1.20）,P〈0.000 01]vs[RR=-0.31,95% CI（-0.55,-0.07）,P〈0.01 ],cough[RR=-3.28,95% CI（-3.92,-2.63）,P〈0.000 01],blood picture[RR=-2.25,95% CI（-3.50,-0.99）,P=0.000 5 ],lung condition[RR=-1.79,95% CI（-2.39,-1.19）,P〈0.000 01]and the total white cell count[RR=-1.58,95% CI（-2.83,-0.33）,P=0.01].No differences were observed in respiratory rate between the two groups.Conclusion：Xuebijing Injection combined with routine therapy exerts better clinical effects on treating hypostatic pneumonia.However,more multicenter and large-scale RCTs should be carried out for further validation due to the restrictions in the quantity and quality of the included studies.
