宁夏贺兰县2008-2015年疑似预防接种异常反应监测数据分析

Analysis on surveillance data of adverse events following immunizatian in Ningxia Helan County 2008-2015

目的了解贺兰县2008-2015年疑似预防接种异常反应(Adverse Events Following Immunization,AEFI)发生特征,评价AEFI监测系统运转情况、疫苗安全性及预防接种服务质量。方法通过国家免疫规划信息管理系统和AEFI信息管理系统分别收集2008-2015年各类疫苗接种剂次及AEFI个案数据,采用描述性流行病学方法进行分析。结果2008-2015年全县共接种17类疫苗667 029剂次,共报告AEFI个案63例,报告单位覆盖率82.35%;AEFI总报告发生率为9.44/10万剂;一般反应报告发生率为3.00/10万剂;异常反应报告发生率为6.00/10万剂;偶合症报告发生率为0.45/10万;一般反应20例(31.75%),异常反应40例(63.49%),偶合症3例(4.76%);0~2岁组较多(77.77%);临床表现多见过敏性皮疹,共28例(44.44%);接种后24 h内发生较多(55.56%)。结论贺兰县AEFI监测敏感性较低,应多措并举提高AEFI监测系统的敏感性。目前使用的一类和二类疫苗安全可靠,预防接种服务质量较高,大大降低了AEFI发生率。

Objective To analyze the characteristics of AEFI years of suspected vaccination abnormal reaction（ hereinafter referred to as 2008-2015） in Helan County,and evaluate the operation of AEFI monitoring system and the quality of vaccination service. Methods Through the national immunization planning information management system and AEFI information management system to collect all kinds of 2008-2015 vaccine inoculation agent and AEFI case data,using descriptive epidemiological methods for analysis. Results From 2008 to 2015,the county was vaccinated with 667 029 doses of vaccine 17 times,63 cases were collected in 2008-2015 form netional AEFI informatain managemrnt system; report unit coverage rate 82. 35%; the estimated total AEFI incidence rate was 9. 44/100 000 doses,General reaction to the report incidence 3. 00/100 000 doses,abnormal reaction reported incidence of 6. 00/100 000 doses,coupling disease reported incidence of 0. 45/100 000; general reaction in 20 cases（ 31. 75%）,abnormal reaction in 40 cases（ 63. 49%）,coupling disease in 3 cases（ 4. 76%）; 1 ~ 2 years more（ 77. 77%）; clinical manifestation see more allergic rash in 28 cases（ 44. 44%）; within 24 hours after inoculation occurred more（ 55. 56%）. Conclusion Helan County lower sensitivity of AEFI surveillance,should take measures to improve the sensitivity of AEFI monitoring system; at present,the use of a class and the two vaccine is relatively safe; the quality of vaccination services in our county is higher,greatly reducing the incidence of AEFI.
