安徽省2016年法定传染病报告质量和管理现状评估

Evaluation on management and quality of notifiable communicable disease reporting in Anhui province,2016

目的了解安徽省法定传染病报告质量和管理现状,发现各级医疗卫生机构在传染病报告管理工作中存在的问题,为进一步提高法定传染病网络直报数据质量提供依据。方法采用分阶段分层随机抽样方法在全省16个市和2个直管县抽取疾病预防控制中心(CDC)和医疗机构开展现场调查。共调查89家机构,抽查法定传染病病例1 268例。结果安徽省各级CDC和医疗机构对传染病报告管理状况日趋完善,法定传染病报告率为95.19%,但各地传染病信息报告管理工作仍存在待改进的问题。不同级别医疗机构报告率差异无统计学意义,淮北地区报告率显著低于江淮之间地区和江南地区。综合评价结果显示,江淮之间地区CDC得分显著低于淮北地区和江南地区(t_1=-4.618,P_1=0.002;t_2=-3.395,P_2=0.013),医疗机构传染病报告管理质量综合评价指数前3位的地市为合肥市(0.9534)、铜陵市(0.9523)、马鞍山市(0.9456)。结论安徽省法定传染病报告管理工作更加规范,报告质量进一步提高。各级各类卫生部门应进一步加强对传染病信息报告工作的管理,完善医疗机构电子病历系统建设,鼓励鼓励有条件的医疗机构积极开展与传染病网络直报系统的直接数据交换功能。

Objective To evaluate the management and quality of notifiable communicable disease reporting in Anhui province and find out existing problems for the further improvement of network direct reporting of communicable disease. Methods The evaluation was conducted by centers for disease control and prevention （CDCs） and hospitals in 16 cities and 2 directly administering counties using multistage stratified cluster sampling. A total of 89 institutions, with 1268 cases of notifiable communicable disease, were investigated. Results The administrative management in all level CDCs and hos- pitals had been gradually optimizing, but the problems still exist. The reporting rate for notifiable communicable disease was 95.19%. There was no significant difference of reporting rate among hospitals in different levels. The reporting rate was the lowest in Huaibei area. Comprehensive evaluation score of quality and management of notifiable communicable disease re- porting in CDCs was lowest in area between Changjiang and Huaihe. The top three cities with highest comprehensive evalua- tion index of quality and management of notifiable communicable disease reporting in medical institutions were Hefei （0. 9534）, Tongling { 0.9523 ） and Maanshan （0.9456 }. Conclusion The more standardized management of notifiable com- municable disease network direct reporting had been conducted and quality of reporting had been greatly improved in Anhui province. It is necessary for all health institutions to continue to strengthen the management of notifiable communicable dis- eases network direct reporting, improve the HIS system, and encourage medical institutions with better facility to carry out direct data exchange with reporting network.
