连续无创血红蛋白监测在小儿手术麻醉中的应用

摘要 目的研究分析连续无创血红蛋白监测在小儿手术麻醉中的应用效果。方法选取2015-02—2016-10间行血管瘤切除术的患儿18例作为研究对象,所有患儿均进行连续无创血红蛋白监测,记录麻醉后、切皮后、术中、术毕时的连续无创血红蛋白监测(Sp Hb)结果,观察Sp Hb与血红蛋白(Hbsatlab)的相关性。结果在术前期、术中稳定期和术中动态期,自变量x为患儿不同时间点Sp Hb值,应变量y为实验室血气分析检测获得的Hbsatlab,经Pearson相关性分析得不同时期Sp Hb与Hbsatlab存在线性相关,方程分别为y=-0.4+1.21x、y=-1.79+1.20x、y=-1.17+1.15x;在术前期、术中稳定期和术中动态期,X轴为围术期各时间点取得Sp Hb与Hbsatlab的均值,Y轴为差值,经Bland-Altman一致性分析,Sp Hb与Hbsatla有良好的一致性。结论连续无创血红蛋白监测在小儿手术麻醉中具有较高的临床应用价值,值得推广应用。 Objective To study the effect of continuous noninvasive hemoglobin monitoring application in pediatric anesthesia.Methods 18 cases under the choice of aneurysm resection in the hospital from February,2015 to October,2016 were selected as the research object. All children had continuous noninvasive hemoglobin monitoring, continuous noninvasive monitoring hemoglobin( Sp Hb) result was recorded after anesthesia,cutting skin,intraoperative and after operation. Correlation of Sp Hb with Hbsatlab was observed. Results In the preoperative period,operative stabilization and intraoperative dynamic stage,the independent variable x values Sp Hb for children with different time points,and dependent variable y measures Hbsatlab through blood gas analysis laboratory Sp Hb. By Pearson correlation analysis in different periods,Sp Hb and Hbsatlab have linear correlation,y =-0. 4 + 1. 21 x,y =-1. 79 + 1. 20 x,y =-1. 17 + 1. 15 x. In the preoperative period,operative period and intraoperative dynamic period,the X axis is Sp Hb and Hbsatlab in average for perioperative each time point,and Y axis is the difference value. Through the Bland-Altman consistency analysis,Sp Hb and Hbsatla have good consistency. Conclusion Continuous noninvasive monitoring hemoglobin in pediatric anesthesia has high clinical value,so it is worthy of popularization and application.
