基于物联网技术社区老年高血压管理模式效果分析

Analysis of effect of community elderly hypertension management mode based on Internet of things technology

目的 :评价基于物联网远程监测技术的老年高血压患者管理模式的效果,探索社区慢性病管理模式。方法 :选择2016年本社区卫生服务中心已被明确诊断、已建立高血压档案、危险评级为中等危险、并至少接受规范管理(每3个月随访1次)1年的老年高血压患者30例,其中男性8例,女性22例,平均年龄(81.3±10.05)岁。评价其使用远程电子家庭血压监测设备6个月后的血压控制水平、血压测量频率、按时服药情况和患者满意度。结果 :使用远程电子家庭血压设备后6个月后,患者的收缩压和舒张压都有明显下降,血压测量频率有明显增加、服药情况和患者满意度均较使用前明显改善,差异均具有统计学意义(P<0.05)。结论 :实施基于物联网技术的新型高血压管理模式,有利于改善高血压患者的自我管理水平,提高血压有效控制水平和满意度。

Objective： To evaluate the effect of remote monitoring technology based on Internet of things in the management of elderly hypertensive patients to explore a new mode of the community chronic disease management. Methods： A total of 30 cases of elderly hypertension who were diagnosed definitely by the community health service center, whose hypertension files were established and their risk rating was moderately dangerous, and who received the standardized management （at least once every three months follow-up） for 1 year in 2016 were selected, and among them there were 8 males and 22 females, with an average age of （81.3±10.05） years. The blood pressure control level, blood pressure measurement frequency, on time medication and satisfaction of the patients were evaluated after 6 months of the use of the remote electronic home blood pressure monitoring device. Results： After using the remote electronic home blood pressure device 6 months later, the systolic and diastolic blood pressures of the patients decreased significantly, and the frequency of blood pressure measurement increased significantly, medication and satisfaction of the patients were significantly improved compared with those before use, and the difference was statistically significant（P〈0.05）. Conclusion： The implementation of new hypertension management mode based on Internet of things technology will help improve the self-management level of patients with hypertension, improve effective control levels of the blood pressure and satisfaction.
