2014—2016年某院临床药师参与临床会诊的调查与分析

Investigation and analysis on clinical pharmacists participating in clinical consultation in our hospital during 2014-2016

目的分析2014—2016年该院临床药师参与的临床会诊工作和效果,为临床药师参与临床会诊的模式、提高会诊意见采纳率做参考。方法采用回顾性分析方法调取该院2014—2016年临床药师参与会诊的12 399例病例,从会诊类型、会诊目的、会诊意见采纳率进行统计分析。结果该院临床药师参与的会诊数量逐年上升,会诊类型主要为科间会诊;会诊目的以抗菌药物会诊为主,会诊比例由2014年的96.45%下降为2016年的86.65%,其他药物会诊比例由2014年的2.84%上升至2016年的7.25%,药物不良反应会诊比例由2014年的0.71%上升至2016年的6.10%;对比排名前十位的内、外科会诊科室,会诊意见接受率为97.8%、95.8%。结论临床药师深入临床参加会诊的数量逐年增多,会诊目的不再局限于抗菌药物的会诊,其他药物及药物不良反应的会诊比例明显增加。外科对抗菌药物会诊的需求较内科更多,内科对其他药物会诊的需求较外科更多,内、外科对会诊意见的采纳率均较高。临床药师可根据内、外科会诊的特点,在明确会诊目的的前提下,发挥有效的会诊作用,提高会诊意见采纳率。

Objective To analyze the clinical consultation work and effect of clinical pharmacists participating in our hospital during 2014-2016 to provide reference for clinical pharmacists participating in clinical consultation mode and increasing the consultation opinion adopting rate. Methods The retrospective analysis method was adopted to extract 12 399 clinical phar-macists participating consultation cases in our hospital during 2014-2016 for conducting the analysis in the aspects of the consul-tation type,consultation purpose and consultation opinion adoption rate. Results The number of clinical pharmacists participating consultation in our hospital was increased year by year and the consultation type was mainly the inter-department consultation.The consultation purpose was dominated by antibacterial drug consultation,its consultation proportion was decreased from 96.45%in 2014 to 86.65% in 2016. The proportion of other drug consultations was increased from 2.84% in 2014 to 7.25% in 2016,and the consultation proportion of adverse drug reactions was increased from 0.71% in 2014 to 6.10% in 2016. The consultation opin-ion adoption rates in the comparison of top 10 internal medicine and surgery departmetns were 97.8% and 95.8% respectively.Conclusion The number of clinical pharmacists participating clinical consultation is increased year by year. The purposes of consultation are no longer limited to the consultation of antibacterial drugs,the consultation proportion of other drugs and adverse drug reactions is significantly increased. The surgical department has more demands for antibacterial drugs consultation than the internal medicine department,while the internal medicine department has more demands for the other drugs consultation than surgical department. The consultation opinion adoption rates of internal medicine delartment and surgical departmentare higher.The clinical pharmacist play effective consultation role to increase the consultation opinion adoption rate according to the internal medicine and surgical consultation characteristics under determining the consultation purpose.
