米诺环素联合根管治疗对牙周牙髓综合征患者牙周指数及细菌感染的影响

Effect of Minocycline Hydrochloride Combined with Root Canal Therapy on Periodontal Index and Bacterial Infection in Patients with Periodontal Syndrome

目的:评价米诺环素联合根管治疗对牙周牙髓综合征患者牙周指数及细菌感染的影响。方法:选取2014年12月-2015年12月在本院就诊的100例牙周牙髓综合征患者,随机将患者分为对照组和试验组,每组50例。试验组使用现代根管治疗方法,根管途径给予米诺环素,同时牙周袋内上盐酸米诺环素;对照组给予传统根管治疗配合根管内封甲醛甲酚(FC)或者樟脑酚(CP),同时牙周袋内上碘甘油。两组患者均于手术前后不同时间点观察菌斑指数(PLI)、探诊后龈沟出血指数(BI)及牙龈指数(GI),并于术后即刻评价细菌学指数。结果:治疗后,两组患者的PLI、BI及GI均显著低于治疗前(P<0.05),且试验组的PLI、BI及GI均明显低于对照组(P<0.05)。治疗后,两组患者的感染率均显著低于治疗前(P<0.05),且试验组感染率低于对照组(P<0.05);试验组的清除率显著高于对照组(P<0.05);试验组患者耐药率为6.0%,显著低于对照组的12.0%(P<0.05)。结论:盐酸米诺环素联合现代根管治疗方法治疗牙周牙髓综合征可有效提高临床效果,值得推广使用。

Objective：To evaluate the effect of Minocycline Hydrochloride combined with root canal therapy on periodontal index and bacterial infection in patients with periodontal syndrome.Method：100 patients with periodontal pulp syndrome in our hospital from December 2014 to December 2015 were selected,they were randomly divided into treatment group and control group,50 cases in each group.Patients in control group were given the traditional root canal therapy with formocresol root canal,while Iodine Glycerol in the periodontal pocket;patients in treatment group were treated with Minocycline Hydrochloride via root canal combined with modern root canal therapy and at the same time minocycline hydrochloride in periodontal pockets.The plaque index,tooth mobility,probing pocket depth,bleeding on probing index and gingival index were observed before and at the right moment after surgery.Result：After treatment,the plaque indexes,bleeding index and gingival index of patients in both groups were significantly lower than those before treatment（P〈0.05）,and in treatment group,they were significantly lower than those in control group（P〈0.05）.After treatment,the infection rates of the two groups were significantly lower than those in control group（P〈0.05）,and the experimental group was significantly lower than that in control group（P〈0.05）,the clearance rate of the treatment group was significantly higher than that of the control group（P〈0.05）,the resistant rate of the experimental group was 6.0%,which was significantly lower than 12.0% of the control group（P〈0.05）.Conclusion：Minocycline Hydrochloride combined with modern root canal therapy can effectively improve the clinical effect of periodontal pulp syndrome,and it is worth popularizing.
