清热化瘀排石颗粒治疗肾下盏结石临床研究

The Clinical Research into Kidney Stone Treated with Granules for Clearing away Heat and Removing Stasis and Stone

目的:观察清热化瘀排石颗粒治疗肾下盏结石的临床疗效。方法:选取80例肾下盏结石患者,均为体外冲击波碎石术(Extracorporeal shock wave lithotripsy,ESWL)术后需继续治疗者,随机分为对照组和治疗组,每组各40例。对照组采用体位排石法,治疗组在对照组的基础上加用清热化瘀排石颗粒。观察两组患者的临床疗效、结石排净时间、VAS评分、肾绞痛发生率及泌尿系感染发生率。结果:治疗组有效率为90.0%,对照组有效率为75.0%,两组患者临床疗效比较,差异具有统计学意义(P<0.05)。治疗组结石排出时间为(12.4±2.2)d,对照组结石排出时间为(22.5±5.6)d,两组患者结石排出时间比较,差异具有统计学意义(P<0.05)。治疗组肾绞痛发生率为17.5%,对照组肾绞痛发生率为35.0%,两组患者肾绞痛发生率比较,差异具有统计学意义(P<0.05)。治疗组VAS评分低于对照组,两组患者VAS评分比较,差异具有统计学意义(P<0.05)。治疗组泌尿系感染发生率为7.5%,对照组泌尿系感染发生率为45.0%,两组患者泌尿系感染发生率比较,差异具有统计学意义(P<0.05)。结论:清热化瘀排石颗粒能有效提高ESWL术后残石的排净率,缩短排净时间,同时也可以大大减少肾绞痛的发生率及VAS疼痛评分,降低泌尿系感染发生率。

Objective： To observe the clinical curative effect of Granules for Clearing away Heat and Removing Stasis and Stone on kidney stone. Methods ： Eighty patients with calculus of inferior calyx of kidney were randomly divided into the control group and the treatment group, with 40 cases in each group. The ones in the control group were treated with postural drainage, while the ones in the treatment group were added Granules for Clearing away Heat and Removing Stasis and Stone based on the treatment of the control group. Then the clinical efficaeies, stone drainage time, VAS score, incidence of renal colic and urinary tract infection rate were observed in the two groups. Results ： The effective rate of the treatment group was 90.0%, while that of the control group was 75%; And the difference between them had statistical significance （P 〈0.05 ）. The stone drainage time in the treatment group was （ 12.4 ± 2.2 ） d, while that of the control group was （22.5 ± 5.6 ） d, and the stone drainage time between the two groups was statistically significant （P 〈0.05 ）. The incidence of renal colic was 17.5% in the treatment group and 35.0% in the control group, and the difference in the incidence of renal colic between the two groups was statistically significant （ P 〈 0.05 ）. The treatment group＇s VAS score was lower than that of the control group, the difference was statistically significant （ P 〈 0.05 ）. The incidence of urinary tract infection in treatment group was 7.5% . while that of the control group was 45% , and the difference was statistically significant （ P 〈 0.05 ）. Conclusion ： Granules for Clearing away Heat and Removing Stasis and Stone can effectively improve the removal rate of residual stones after ESWL, shorten the cleaning time. and also greatly reduce the incidence of renal colic and VAS pain score, ,and reduce the incidence of urinary tract infection.
