超声联合多层螺旋CT血管成像对颈部动脉溃疡斑块的诊断价值分析

Diagnostic value of ultrasound combined with multi-slice spiral CT angiography in carotid artery plaque

目的探讨彩色多普勒超声联合CT血管成像对颈部动脉溃疡斑块的诊断价值。方法选取本院2014年9月至2016年6月收治的120例颈部动脉溃疡斑块患者作为研究对象,患者均经DSA确诊为颈部动脉溃疡斑块。对患者行超声和多层螺旋CT血管成像检查,比较超声、多层螺旋CT血管成像对颈部动脉狭窄和溃疡斑块的检出情况以及诊断符合率。结果超声检出颈动脉轻度狭窄数、中度狭窄数显著少于CT血管成像(P<0.05);超声检出颈动脉重度狭窄数与CT血管成像比较,无显著差异(P>0.05)。超声检出颈动脉溃疡斑块数量显著少于CT血管成像,差异具有统计学意义(P<0.05)。超声的诊断符合率低于CT血管成像(P<0.05)。结论超声与多层螺旋CT血管成像对颈部动脉溃疡斑块诊断各具优势,两者联合可实现优势互补,进一步提高诊断准确性。

Objective To evaluate the value of color Doppler ultrasonography combined with CT angiography in the diagnosis of carotid artery plaque. Methods One hundred and twenty cases of patients with carotid artery plaque admitted to our hospital from September 2014 to June 2016 were selected, which were diagnosed with digital subtraction angiography （DSA）. The patients underwent ultrasound and multi-slice spiral CT angiography, and then the detection situation of carotid artery stenosis and ulcer plaque and diagnose accordance rates between ultrasound and multi-slice spiral CT angiography were compared. Results The detection number of mild stenosis, moderate stenosis of ultrasound were significantly less than those of CT angiography, the differences were statistically significant （P〈0.05）; the detection number of severe stenoses of ultrasound had no significant difference with that of CT angiography （P 〉0.05）. The detection number of carotid atherosclerotic plaques of ultrasound was significantly less than that of CT angiography, the difference was statistically significant （P〈0.05）. The diagnosis accordance rates of ultrasound was significantly lower than that of CT angiography （P〈 0.05）. Conclusion Ultrasonography and multi-slice spiral CT angiography of carotid artery plaque ulcer have their own advantages, the combination of the two method can achieve complementary advantages, so as to further improve the diagnosis accuracy.
