摘要
目的探讨彩色多普勒超声筛查孕中晚期筛查胎儿畸形的价值研究。方法选自我院于2016年1月至2016年12月期间进行产前检查中孕中晚期产妇2784例。采用彩色多普勒超声诊断仪,探头频率为2.5MHz至5.0MHz。取孕妇侧卧位或者仰卧位,行多方位详细检查。结果本组中孕中晚期产妇2784例产前检出胎儿畸形40例。其中心脏畸形9例、脑积水5例、无脑儿4例、唇、腭裂2例、脊椎畸形8例、四肢畸形9例、泌尿道畸形3例;本组40例胎儿畸形中,漏诊5例,包括心脏畸形2例、脊椎畸形2例、四肢畸形1例;心脏畸形与妊娠结局符合率为77.78%,脑积水与妊娠结局符合率为100.00%,无脑儿与妊娠结局符合率为100.00%,唇、腭裂与妊娠结局符合率为100.00%,脊椎畸形与与妊娠结局符合率为75.00%,四肢畸形与妊娠结局符合率为97.50%,泌尿道畸形与妊娠结局符合率为100.00%。畸形胎儿与妊娠结局符合率为87.50%。结论彩色多普勒超声筛查能够有效地筛查孕中晚期胎儿畸形,且可作为一种妊娠早期漏诊的检查手段,为临床诊断胎儿畸形提供参考价值。
Objective:To investigate the value of color Doppler ultrasonography in screening fetal malformation in the middle and late pregnancy. Methods:From January 2016 to December 2016 in our hospital during the prenatal examination of 2784 cases of advanced maternal. Using color Doppler ultrasound diagnostic instrument,probe frequency of 2.5 MHz to 5 MHz. Take a pregnant woman lying on the side or supine position,multi-directional detailed inspection. Results:In this group,2784 cases of prenatal fetal malformation were detected in the middle and late stage of pregnancy in this group(n=40). The cardiac malformation in 9 cases,5 cases of hydrocephalus,4 cases of anencephaly,2 cases of cleft lip and palate,8 cases of spinal deformity,9 cases of limb deformities,3 cases of urinary tract malformation;40 cases of this group of fetal malformation,5 cases of misdiagnosis,including heart malformation in 2 cases,2 cases of limb deformities,spinal deformity 1 cases of cardiac malformations and pregnancy outcome;the coincidence rate is 77.78%,hydrocephalus and pregnancy outcomes the coincidence rate is 100%,anencephaly and pregnancy outcomes the coincidence rate is 100%,and the pregnancy outcome of cleft lip and palate coincidence rate was 100%,and vertebral deformity and pregnancy outcome coincidence rate was 75%,limb deformities and pregnancy outcome coincidence rate was 97.50% urinary tract abnormalities and pregnancy outcome,the coincidence rate is 100%. The coincidence rate of fetal malformation and pregnancy outcome was 87.50%. Conclusion:Color Doppler ultrasound screening can effectively screening for fetal abnormalities in the middle and late pregnancy,and can be used as a means of early pregnancy missed diagnosis,provide a reference value for the clinical diagnosis of fetal malformations.
