临床药师参与一例孕25周胎膜早破患者使用宫缩抑制剂的病例分析

Case analysis of clinical pharmacists participating in the tocolytics therapy for a 25-week pregnant women with preterm premature rupture of membranes

目的探讨临床药师在未足月胎膜早破患者治疗与药学监护中的工作要点。方法临床药师参与1例孕25周胎膜早破患者的抑制宫缩、延长孕周治疗用药方案的调整及用药监护。结果临床药师根据患者特点,协助医师选择最适合的宫缩抑制剂-利托君,并配合全程化药学监护,取得了良好的效果,且无药品不良事件(ADE)发生。结论个体化给药方案的设计、分析与用药监护是临床药师开展药学服务工作的切入点。

Objective To explore the key working points of clinical pharmacists in the treatment and pharmaceutical care for patients with preterm premature rupture of membranes.Methods Clinical pharmacists participated in the regimen adjustment and supervision for prevention of adverse drug events（ADE） in a case of 25-week pregnant women with preterm premature rupture of membranes.Results Clinical pharmacists effectively assisted doctors in rational selection of tocolytics according to the characteristics of the patient and carried out pharmaceutical care in the whole therapy,which achieved a good therapeutic effect without ADE.Conclusion Design of individualized medication regimen and medication supervision should be the breakthrough point of clinical pharmacists.
