胜利油田社区中老年人群冬季维生素D水平营养状况调查 被引量:2

Investigation of nutritional status of vitamin D in middle aged and old people in Shengli oil field in winter

摘要 目的调查滨南油区中老年人群冬季维生素D水平营养状况,为促进中老年人群预防骨质疏松、心血管疾病、糖尿病,提高机体免疫力等身体健康问题提供科学依据。方法采用随机整群抽样方法,收集2015年11月-2016年1月来自建翔医院35~70岁中老年人群并且在本地区居住5年以上的小区居民体检者血液标本475例,测量身高、体重计算出体重指数,腹部肝胆B超记录脂肪肝情况,罗氏Cobas e411电化学发光分析法测定血清总25-羟基维生素D(25OHD)的量,按年龄、性别分层分析本地区中老年人群维生素D营养状况、25OHD在不同脂肪肝中的分布情况及与体重指数的相关性。结果 (1)滨南油区35~70岁中老年人群血清总25OHD平均浓度(12.31±6.54)ng/ml,严重缺乏、缺乏、不足和充足分别占45.0%、39.8%、14.1%和1.1%。(2)男性25OHD水平明显高于女性,差异有统计学意义(t=11.524,P<0.05)。女性缺乏率(96.0%)明显高于男性(72.3%),差异有统计学意义(χ2=51.674,P<0.05)。(3)各年龄段血清25OHD值差异无统计学意义(F=2.572,P>0.05)。男性组各年龄段血清25OHD值差异无统计学意义(F=0.642,P>0.05)。女性46~55岁组25OHD值较35~45岁组和56~70岁明显偏低,差异有统计学意义(P<0.05)。(4)25OHD在脂肪肝中的分布差异无统计学意义(F=0.196,P>0.05)。超重组25OHD值较非超重组明显低,差异有统计学意义(t=4.309,P<0.05),并且25OHD与体重指数间有明显的相关性(r=0.114,P<0.05)。结论滨南油区35~70岁中老年人群冬季存在较严重的25OHD缺乏状况,25OHD营养状况男性优于女性,46~55岁更年期女性维生素D水平更低,超重者比非超重者有较低的维生素D水平。 Objective To investigate nutritional status of vitamin D in middle aged and old people aged 35-70 years old in Binnan oilfield in winter,and provide a scientific basis for promoting middle aged and old people to prevent osteoporosis,cardiovascular disease,diabetes and improving immunity. Methods Totally 475 subjects aged 35-70 years old and living 5 years above in this region were collected by cluster random sampling method from November,2015 to January,2016 in Jian Xiang hospital. Height and weight were measured to calculate body mass index. Fatty liver was recorded from abdominal hepatobiliary ultrasound. Roche Cobas e411 electrochemical luminescence method was used to detect serum total 25 OHD concentration. Then according to age and gender stratification,vitamin D nutritional status was analyzed,distribution of 25 OHD in different fatty livers and body mass index was analyzed. Results( 1) The average concentration of serum total 25 OHD was( 12. 31±6. 54) ng/ml in middle aged and old people in Binnan oilfield. The serious lack,lack,insufficient and adequate about vitamin D accounted for 45. 0%,39. 8%,14. 1% and 1. 1%.( 2) Male 25 OHD level was significantly higher than than of female,the difference was statistically significant( t = 11. 524,P〈0. 05). Female lack rate( 96%) was significantly higher than that of male( 72. 3%),the difference was statistically significant( χ^2= 51. 674,P〈0. 05).( 3) There was no significant difference in serum 25 OHD concentration between different ages( F = 2. 572,P〈0. 05). There was no significant difference in serum 25 OHD concentration between the male groups( F = 0. 642,P〈0. 05). 25 OHD concentration in female 46-55 years old group was significantly lower than 35-45 years old group and 56-70 years old group,significant difference( P〈0. 05).( 4) There was no significant difference in the distribution of 25 OHD in fatty liver( t = 4. 309,P〈0. 05). There was significant correlation between vitamin D level and body mass index( r = 0. 114,P〈0. 05). Conclusion The middle aged and old people have serious vitamin D lack in Binnan oilfield. 25 OHD nutritional status in male is better than female. 46-55 years old menopause women vitamin D level are lower. Overweight have lower vitamin D level than non overweight.
