磁共振IVIM技术临床应用稳定性分析 被引量:4

Reliability of Intravoxel Incoherent Motion Diffusion Weighted Imaging in Different Part of Body

摘要 目的探讨体素内不相干运动扩散加权成像(IVIM-DWI)技术在不同部位应用的稳定性。方法分别对60例患者包括20例正常表现脑、肝脏及女性盆腔进行3.0 T GE磁共振8b值DWI序列扫描,使用工作站计算相关参数包括慢速ADC(D)、快速ADC(D~*)、灌注相关体积分数ADC(f),每个部位选取3个观测点,脑区包括右侧额叶脑灰质、白质及基底节,肝脏为肝脏顶部、左叶及右叶,盆腔为子宫体肌、臀肌、髂骨骨髓,放置感兴趣区测量,分别计算比较各观测点的D、D~*、f值的变异系数。结果各部位D、D~*、f值变异系数分别为:脑白质0.081、0.299、0.341;脑灰质0.092、0.292、0.297;基底节0.098、0.573、0.333;肝顶0.277、0.545、0.382;肝右叶0.324、0.553、0.352;肝左叶0.367、0.499、0.258;子宫肌壁0.096、0.629、0.197;盆腔肌肉0.101、0.945、0.95;髂骨髓腔0.19、0.481、0.365。9个测量点中D值的变异系数在三个参数中相对较低;三个部位中,脑组织3个观察点的D值及D~*值变异系数较低。结论脑组织的IVIM技术总体应用的稳定性优于盆腔及肝脏;各部位IVIM相关参数中,D值的稳定性好于D~*及f。 Objective To observe the variation of Intravoxel incoherent motion diffusion weighted imaging( IVIM-DWI)in different parts of body. Methods Multi-b-value( 7b) DWI sequences were applied on 60 patients which include 20 normal brain,20 normal liver and 20 normal female pelvis using GE 3. 0T scanner. Three parameters of IVIM,which are true diffusion( D),perfusion related diffusion( D^*),and fraction of perfusion( f) were calculated using AW workstation.Three points were picked on each exam part for measurement which are gray and white matter of right frontal lobe,basal ganglia,top of liver,right and left lobe of liver,muscle of uterus,bone marrow of pelvis and muscle of pelvis. The coefficient of variation( CV) of these parameters was calculated and compared to evaluate the reliability. Results The CVs of D,D^*and f in 9 locations are followed: white matter of brain( 0. 081、0. 299、0. 341),gray matter of brain( 0. 092、0. 292、0. 297),basal ganglia( 0. 098、0. 573、0. 333),top of liver( 0. 277、0. 545、0. 382),right lobe of liver( 0. 324、0. 553、0. 352),left lobeof liver( 0. 367、0. 499、0. 258),muscle of uterus( 0. 096、0. 629、0. 197),muscle of pelvis( 0. 101、0. 945、0. 95),bone marrow of pelvis( 0. 19、0. 481、0. 365). The CV of D was lower than D^* and f in almost all 9 locations. In brain,almost all CVs of D and D-* in three locations were lower than that in liver and female pelvis. Conclusion IVIM technique has good reliability in brain than in liver and female pelvis. The most reliable IVIM parameters is D.
