溃疡性结肠炎患者肠道菌群紊乱与TLR/NK-kB、炎症介质表达的关系 被引量:16

摘要 目的:研究溃疡性结肠炎(UC)患者肠道菌群紊乱与TLR/NK-kB、炎症介质表达的关系。方法:选择在我院诊断为UC的患者,活动期UC患者和缓解期UC患者纳入AUC组和RUC组;另取同一时期在延安市人民医院体检发现结肠息肉的患者作为对照组。采集粪便并检测菌群数量,采集病灶黏膜并检测TLR/NK-kB、炎症介质的表达量,采集血清并检测炎症介质的含量。结果:AUC组和RUC组患者粪便中双歧杆菌、嗜酸乳杆菌、拟杆菌的数目显著低于对照组,粪便中大肠埃希菌、肠球菌的数目以及病变肠黏膜中TLR2、TLR4、TLR5、TLR9、NK-kB、HMGB1、TNF-α、IL-6、IL-17的mRNA表达量以及血清中HMGB1、TNF-α、IL-6、IL-17的含量显著高于对照组;AUC组粪便中双歧杆菌、嗜酸乳杆菌、拟杆菌的数目显著低于RUC组,粪便中大肠埃希菌、肠球菌的数目以及病变肠黏膜中TLR2、TLR4、TLR5、TLR9、NK-kB、HMGB1、TNF-α、IL-6、IL-17的mRNA表达量以及血清中HMGB1、TNF-α、IL-6、IL-17的含量显著高于RUC组;病变肠黏膜中TLR2、TLR4、TLR5、TLR9、NKkB、HMGB1、TNF-α、IL-6、IL-17的mRNA表达量以及血清中HMGB1、TNF-α、IL-6、IL-17的含量与双歧杆菌、嗜酸乳杆菌、拟杆菌的数目呈负相关,与大肠埃希菌、肠球菌的数目呈正相关。结论:溃疡性结肠炎患者肠道菌群紊乱能够通过激活TLR/NK-kB通路来增加炎症介质的表达。 Objective:To study the relationship of intestinal flora disorder with TLR/NK-kB and inflammatory mediator expression in patients with ulcerative colitis(UC).Methods:Patients who were diagnosed with UC in Yan'an People's Hospital were selected,patients with active UC and patients with remission UC were included as the AUC and RUC group;patients who were diagnosed with colonic polyps in Yan'an People's Hospital through physical examination during the same period were selected as the control group.Feces was collected to test the flora number,mucosa lesion was collected to determine the expression of TLR/NK-kB and inflammatory mediators,and serum was collected to detect the levels of inflammatory mediators.Results:Bifidobacterium bifidum,Lactobacillus acidophilus and bacteroides counts in feces of AUC group and RUC group were significantly lower than those of control group while Escherichia coli and Escherichia enterococcus number in feces,TLR2,TLR4,TLR5,TLR9,NK-kB,HMGB1,TNF-α,IL-6and IL-17 mRNA expression in intestinal mucosa lesion as well as HMGB1,TNF-α,IL-6and IL-17 levels in serum were significantly higher than those of control group;Bifidobacterium bifidum,Lactobacillus acidophilus and bacteroides counts in feces of AUC group were significantly lower than those of RUC group while Escherichia coli and Escherichia enterococcus number in feces,TLR2,TLR4,TLR5,TLR9,NK-kB,HMGB1,TNF-α,IL-6and IL-17 mRNA expression in intestinal mucosa lesion as well as HMGB1,TNF-α,IL-6and IL-17 levels in serum were significantly higher than those of RUC group;TLR2,TLR4,TLR5,TLR9,NK-kB,HMGB1,TNF-α,IL-6and IL-17 mRNA expression in intestinal mucosa lesion as well as HMGB1,TNF-α,IL-6and IL-17 levels in serum were negatively correlated Bifidobacterium bifidum,Lactobacillus acidophilus and bacteroides counts,but positively correlated with Escherichia coli and Escherichia enterococcus counts.Conclusion:The intestinal flora disorder in patients with ulcerative colitis can increase the expression of inflammatory mediators by activating the TLR/NK-kB pathway.
