摘要
目的探讨超声光散射乳腺成像(OPTIMUS)系统对乳腺肿瘤的诊断价值。方法回顾性分析经病理证实的321例乳腺肿瘤(恶性肿瘤12例、良性肿瘤309例)的OPTIMUS系统、彩色多普勒超声以及钼靶X线资料,分别比较三者并与病理诊断进行一致性分析,评价OPTIMUS对乳腺肿瘤的诊断价值。结果 OPTIMUS系统与彩色多普勒超声的敏感性差异无统计学意义(P=1.00),两者特异性(P=0.044)及准确率(P=0.035)差异有统计学意义;OPTIMUS系统与钼靶X线的敏感性(P=0.037)、特异性(P=0.034)与准确率(P=0.004)差异均有统计学意义。三者与病理诊断的一致性检验kappa值分别为1.00、0.86、0.54;阳性预测值分别为0.40、0.37、0.32;阴性预测值分别为1.00、0.97、0.87。结论 OPTIMUS系统对诊断乳腺肿瘤的敏感性、特异性和准确率较高,与病理诊断的一致性较好,与彩色多普勒超声和钼靶X线相结合可以提高乳腺患者的检出率,降低漏诊率。
Objective To investigate the diagnostic value of diffused optical tomography with ultrasound(OPTIMUS)in breast cancer.Methods The data of OPTIMUS,ultrasound and X-ray examination of 321 breast tumors(309benign breast tumors,12 malignant breast tumors)were analyzed retrospectively and compared with pathological results.Results There are no statistical difference in sensitivity between OPTIMUS and ultrasonic(P=1.00),but there are statistical difference in specificity(P=0.044)and accuracy(P=0.035).Between OPTIMUS and X-ray examination,there are statistical difference in sensitivity(P=0.037),specificity(P=0.034)and accuracy(P=0.004).Consistency of the three with pathological results is 1.00、0.86、0.54,positive predictive value is 0.40、0.37、0.32,negative predictive value is1.00、0.97、0.87.Conclusion OPTIMUS have higher value in sensitivity,specificity and accuracy and better consistency with pathological diagnosis in breast cancer.Combining with ultrasound and X-ray examination can improve the detection rate and reduce the missed diagnosis rate in breast cancer.
