摘要 目的探讨Haste动态扫描技术在颞下颌关节紊乱中的应用价值。方法回顾性分析经临床随访或手术后证实的颞下颌关节紊乱病(TMD)患者48例共96个颞下颌关节的MRI表现,将常规静态MRI图像与Haste动态扫描图像进行对比分析。结果 Haste动态扫描序列可以清晰显示颞下颌关节的运动轨迹,显示运动过程中关节盘及髁状突的位置改变、关节盘的形态改变及关节腔积液,对于关节盘前后移位、关节活动度异常、关节腔积液均能正确诊断,本组48例96个颞下颌关节中83个Haste动态扫描与常规静态MRI扫描诊断结果一致,诊断符合率为86.5%。结论 Haste动态扫描序列在发挥其动态显示颞下颌关节运动过程优势的同时对于大多数颞下颌关节紊乱病可正确诊断,在临床工作中可作为常规MRI检查的补充。 Objective To explore the value of Haste sequence in temporomandibular joint disorders(TMD). Methods The MRI imaging of 96 temporomandibular joints in 48 patients with TMD confirmed by clinical followup or after surgery were analyzed retrospectively,the routine static MRI images and Haste images were compared and analyzed. Results Haste sequence could clearly show the trajectory of the temporomandibular joint,show the position change of articular disc and condyle,the morphological changes of disc,and joint effusion. For the anterior or posterior disc displacement,joint mobility abnormalities,joint effusion could be correctly diagnosed. In the 48 cases of 96 temporomandibular joints,83 Haste sequence were consistent with the routine static MRI, and the diagnostic accuracy was 86.5%. Conclusion Haste sequence in its dynamic display of the temporomandibular joint movement advantage at the same time for the majority of temporomandibular disorders can be diagnosed correctly,it can be used as a supplement to routine MRI in clinic.
