急性心肌梗死患者PCI治疗前后脂联素动态变化与左心室收缩功能的关系 被引量:5

Relationship between Adiponectin Dynamic Changes and Left Ventricular Systolic Function in Patients with Acute Myocardial Infarction before and after Percutaneous Coronary Intervention

摘要 【目的】探讨急性心肌梗死(AMI)患者经皮冠状动脉介入术(PCI)前后脂联素(APN)动态变化与左心室收缩功能的关系。【方法】选择2015年1月至2016年1月于本院行择期PCI治疗的AMI患者100例作为观察组,选取性别、年龄匹配的体检健康者90例作为对照组。观察组于PCI术前、术后1个月、术后3个月及对照组检测APN水平,并采用二维斑点追踪成像技术检测观察组左心室收缩功能[左室舒张末期客积(LVESV)、左心室收缩末期内径(LVESD)、左心室射血分数(LVEF)],并分析APN与LVESV、LVESD、LVEP的相关性。【结果】观察组术前及术后1、3个月APN低于对照组,差异有统计学意义(P〈0.05);观察组术后1、3个月APN高于术前(P〈0.05),且术后3个月高于术前1个月,差异有统计学意义(P〈0.05)。观察组患者术后1、3个月I,VESV、LVESD低于术前,LVEF高于术前(P〈0.05),且术后3个月LVESV、LVESD低于术后1个月,LVEF高于术后1个月,差异有统计学意义(P〈0.05)。Pearson相关分析显示,血清APN与LVESV、LVESD呈负相关(r=-0.716、-0.618,P〈0.05),与LVEF呈正相关关系(r=0.689,P〈0.05)。【结论】AMI患者PCI后APN水平与左心室收缩功能变化密切相关。 [Objective]To explore the relationship between the adiponeetin (APN) dynamic changes and left ventricular systolic function in patients with acute myocardial infarction (AMI) before and after percutane- ous coronary intervention (PCI). [Methods] A total of 100 cases of AMI patients treated with elective PCI in our hospital from January 2015 to January 2016 were selected as the observation group, and 90 healthy exam- iners with the matching gender and age were selected as the control group. The levels of APN were detected in the both group before PCI as well as 1 month and 3 months after operation. The two-dimensional speckle tracking imaging was used to detect the left ventricular systolic function [left ventricular end diastolic volume (LVESV), left ventricular end systolic diameter (LVESD) and left ventricular ejection fraction (LVEF) of the observation group. The correlation between APN and LVESV, LVESD, LVEF was-analyzed. [Results] The APN level before operation, i month and 3 months after operation were lower in the observation group than those in the control group ( P〈0.05). The APN level of the observation group was higher at 1 month and 3 months after operation than it was before operation ( P〈0.05), and the APN level at 3 months after operation was higher than that at 1 month before operation ( P 〈0.05). The levels of LVESV and LVESD at 1 month or 3 months after operation in the observation group were lower than those before operation, while the LVEF level was higher than that before operation (P 〈0.05).The levels of LVESV and LVESD at 3 months after operation were lower than those at 1 month after operation, while the LVEF level was higher than that at 1 month after operation ( P 〈0.05). Pearson correlation analysis showed that serum APN was negatively correlated with the LVESV and LVESD (r ted with LVEF (r=0.689, P〈0.05).
