慢性阻塞性肺疾病急性加重期糖皮质激素治疗疗效观察

Curative effect observation of glucocorticoid on acute exacerbation of chronic obstructive pulmonary disease

目的探究慢性阻塞性肺疾病急性加重期（AECOPD）采用糖皮质激素治疗的lI缶床疗效。方法纳入滨州医学院烟台附属医院2014年5月至2016年5月收治人院的AECOPD患者60例，随机分为对照组和观察组，每组30例。对照组患者采用特布他林实施治疗，观察组患者在采用特布他林的基础上联合糖皮质激素进行治疗，观察2组患者的治疗效果、动脉血气指标以及肺通气功能情况。结果糖皮质激素联合特布他林治疗组的SpO2、PaO2、PaCO2、pH值等动脉血气指标显著改善，临床治疗有效率高达93．3％，与对照组患者的73．3％相比，差异有统计学意义。结论对AECOPD患者采取特布他林联合糖皮质激素治疗的临床疗效显著，改善患者呼吸状况，有效缓解患者病情，值得推广。

Objective To investigate the clinical curative effect of glucocorticoid on patients with acute exacerbation of chronic obstructive pulmonary disease（AECOPD） aggravating period. Methods Sixty AECOPD patients in Yantai Affiliated Hospital of Binzhou Medical University from May 2014 to May 2016 were selected and randomly divided into control group and observation group （ n = 30）. All patients in control group were treated with terbutaiine, and patients in the observation group were treated with glucocorticoid on the basis of the control group, the symptoms of the patients were examined before treatment, and the arterial blood gas parameters and clinical effects of the two groups were observed. Results After treatment, the total effective rate in patients of observation group was 93.3%, which is significantly higher than that of control group （73.3%, P 〈0.05）. And oxygen saturation, oxygen tension, partial pressure of carbon dioxide and pH value, such as arterial blood gas index in observation group were significantly better than that of control group （ P 〈0.05）. Conclusions Application of glucocorticoid in the clinical treatment of AECOPD patients aggravating stage, can effectively improve the patient＇s artery blood gas index, the clinical curative effect is distinct, worthy of promotion.
