Path analysis on the effect of social support and resilience on depression of renal transplant patients

摘要 目的 探讨社会支持和心理弹性对肾移植患者抑郁的影响路径及关系效应.方法 采用方便抽样法收集2015年1月—2016年4月在南京大学医学院附属鼓楼医院泌尿外科住院和规律门诊随访的初次成功接受肾移植的患者104例,采用抑郁自评量表、社会支持评定量表和心理弹性量表进行测评.采用Amos 7.0软件进行路径分析.结果 患者抑郁、社会支持和心理弹性得分分别为(50.15〈11.23)、(43.87〈7.21)、(71.32〈18.72)分;且上述三变量间存在相关性(r=0.443-0.562,P〈0.05).社会支持对抑郁有直接负效应,并通过心理弹性对抑郁产生间接影响,总效应值为-1.24.心理弹性对抑郁有直接负效应,亦是社会支持与抑郁的中介变量,总效应值为-0.71.结论 肾移植患者社会支持和心理弹性水平尚佳,但存在抑郁.抑郁变量受到社会支持和心理弹性的直接、间接影响.护士可从各环节制定干预措施,改善抑郁状况. Objective To explore the effect of social support and resilience on depression of renal transplant patients and its relationship effect.Methods A total of 104 patients who underwent renal transplant successfully for the first time in Nanjing Drum Tower Hospital during January 2015 to April 2016 were recruited into the study. They were evaluated by self-rating depression scale, social supporting rating scale and resilience scale for patients. Analysis of Moment Structures (AMOS) version 7.0 software was used to conduct the path analysis.Results The scores of patients' depression, social support and resilience were (50.15〈11.23), (43.87〈7.21) and (71.32〈18.72), and there was correlationship among them (r=0.443-0.562,P〈0.05). Social support revealed direct negative effect on depression and affected depression indirectly via resilience. The total coefficients was -1.24. Resilience was a direct negative factor of depression and it was also intermediary variable between social support and depression. The total coefficients was -0.71.Conclusions The level of social support and resilience are fine for renal transplant patients, however, they have depression. Depression is directly or indirectly affected by social support and resilience. Nurses can make intervening measures in different links to improve patients' depression status.
