肾上腺髓质素、内皮素-1及一氧化氮在妊娠高血压患者血清中的表达及检测意义

Expression and significance of adrenomedullin,endothelin-1 and nitric oxide in serum of patients with hypertensive disorders pregnancy

目的:探讨肾上腺髓质素(ADM)、内皮素-1(ET-1)及一氧化氮(NO)在妊娠高血压(HDP)患者血清中的表达及检测意义。方法:77例HDP患者根据病情程度分为HDP组(n=25)、轻度子痫前期组(n=24)及重度子痫前期组(n=28),同期选择30例正常孕妇为正常妊娠组,30例正常未妊娠妇女为对照组。采用酶联免疫吸附法检测各组血清ADM、ET-1及NO表达水平。结果:HDP组和正常妊娠组血清ADM、ET-1表达水平明显高于对照组(P<0.05),而血清NO表达水平明显低于对照组(P<0.05),且HDP组血清ADM、ET-1表达水平明显高于正常妊娠组,而血清NO表达水平明显低于正常妊娠组(P<0.05);随病情程度逐渐加重,HDP患者血清ADM、ET-1表达水平明显升高(P<0.05),而血清NO表达水平明显降低(P<0.05);血清ADM表达水平与ET-1表达水平呈正相关(r=0.752,P<0.05),而与NO表达水平呈负相关(r=-0.642,P<0.05)。结论:血清ADM、ET-1及NO检测有助于早期诊断HDP和判断病情严重程度,具有较高的临床价值。

Objective： To evaluate the expression of adrenomedullin （ADM）, endothelin-1 （ET-1） and nitric oxide （NO） in patients with hypertensive disorders pregnancy （HDP）,and to investigate the clinical significance.Methods：Seventy-seven patients with HDP were divided into hypertension group （n =25）,mild preeclampsia group （n =24） and severe preeclampsia group （n = 28） according to the different levels of illness severity. Thirty cases of normal pregnant women were selected as normal pregnancy group, 30 cases of normal non-pregnant women were selected as control group.The levels of serum ADM,ET-1 and NO were detected by enzyme linked immunosorbent assay.Results： The expression level of serum ADM,ET-1 of HDP group and normal pregnancy group were significantly higher than those of control group （P〈0.05） ,and the expression level of serum NO was significantly lower than that of control group （P〈0.05）.The expression level of serum ADM,ET-1 of HDP group were significantly higher than those of normal pregnancy group, and the expression level of serum NO was significantly lower than that of normal pregnancy group （P〈0.05）.With the severity of disease gradually increase, the expression level of serum ADM, ET-1 of HDP patients significantly increased （P〈0.05）,and the expression level of serum NO decreased significantly（P（0.05）. The expression level of ET-1 was positively correlation to the expression level of serum ADM （r=0.752,P〈0.05）, and negative correlation to the expression level of NO（r=-0.642,P〈 0.05）.Conclusion：The detection of serum ADM,ET-1 and NO is helpful for early diagnosis of HDP and judging the severity of the disease,and has a high clinical value.
