紫杉醇联合顺铂与奥沙利铂治疗晚期食管癌的疗效比较研究及其安全性

Comparative effectiveness research and the safety of paclitaxel combined with cisplatin and oxaliplatin in treating advanced esophageal cancer

目的比较紫杉醇联合顺铂与奥沙利铂治疗晚期食管癌的临床疗效及安全性。方法选取盐城市射阳县人民医院2013年1月—2015年10月收治的晚期食管癌患者70例,根据化疗方案不同将患者分为PC组38例与PO组32例。PC组患者采取紫杉醇+顺铂方案化疗,PO组采取紫杉醇+奥沙利铂方案化疗,两组患者均持续化疗4个周期。比较两组患者临床疗效及毒副作用发生情况。结果化疗4个周期后,两组患者治疗有效率及治疗控制率比较,差异无统计学意义(P>0.05)。两组患者血象异常、肌肉关节疼痛、肝肾功能损伤发生率比较,差异无统计学意义(P>0.05);PO组患者消化道不适发生率低于PC组(P<0.05)。结论紫杉醇联合顺铂与奥沙利铂治疗晚期食管癌的短期疗效相当,但紫杉醇联合奥沙利铂化疗方案对患者胃肠道影响较小。

Objective To compare the clinical efficacy and safety of paclitaxel combined with cisplatin and oxaliplatin in treating advanced esophageal cancer. Methods A total of 70 cases of advanced esophageal cancer patients were selected from January 2013 to October 2015 in Sheyang County People＇s Hospital of Yancheng City,which were divided into PC group（ n= 38） and PO group（ n = 32） according to different chemotherapy regimens. PC group was given paclitaxel combined with cisplatin,PC group was given paclitaxel combined with oxaliplatin,both groups were treated for 4 courses. The clinical effect and the toxic and side effects were compared between the two groups. Results After chemotherapy of 4 courses,no significant differences of total efficiency rate or treatment control rate was found between the two groups（ P〉0. 05）. No significant differences of hemogram abnormality rate,pain in the joint of muscles,damage rate of liver and kidney function was found between the two groups（ P〉0. 05）; but the incidence of gastrointestinal of PO group was significantly lower than PC group（ P〈0. 05）. Conclusion Paclitaxel combined with cisplatin have a similar clinical effect with oxaliplatin in treating advanced esophageal cancer,but paclitaxel combined with oxaliplatin have less gastrointestinal adverse reaction.
