慢性胰腺炎合并胰管结石的外科治疗策略

Surgical Treatment of Chronic Pancreatitis Complicated with Pancreatic Duct Stones

慢性胰腺炎(Chronic Pancreatitis,CP)是指各种原因导致的胰腺不可逆性的慢性炎症,常伴有胰腺内、外分泌功能不同程度的损害。其发病率逐年上升,早期诊断困难,往往当发生慢性胰腺炎并发症后,如胰管结石、胆道梗阻、十二指肠梗阻、胰腺假性囊肿、胰源性门静脉高压及胰源性胸腹水等才能得到准确的诊断,其中合并胰管结石较多见,此时,非手术治疗效果并不理想,绝大多数患者需要外科手术干预才能改善症状,提高患者的生活质量。笔者复习了国内外大量相关文献资料,对慢性胰腺炎合并胰管结石的外科治疗策略进行总结和论述,旨在提高临床医生对慢性胰腺炎合并胰管结石外科治疗方法的认识,最大程度地改善患者的症状,提高患者的生活质量。

Chronic Pancreatitis （CP） is a chronic and irreconcilable inflammation caused by various causes, with pancreatic endocrine or exocrine function damage. Incidence of CP increases year by year and early diagno- sis is difficult. CP often can be diagnosed accurately after the occurrence of chronic pancreatitis complications, such as pancreatic duct stones, biliary obstruction, duodenal obstruction, pancreatic pseudocyst, pancreatic por- tal vein hypertension and pancreatic pleural effusion or ascites and so on, in which pancreatic duct stones are more common. In this time, non - surgical treatment is not ideal, the vast majority of patients need surgical in- tervention to relieve symptoms and improve quality of life. I reviewed a large number of relevant literature at home and abroad, and made summary and discussion on the Surgical treatment strategy of chronic pancreatitis with pancreatic duct stones, aiming to raise the understanding on surgical treatment strategy of chronic pancreatitis with pancreatic duct stones, and then to relieve patient＇s symptoms and improve patient＇s quality of life to the greatest extent.
