中药开内服外敷联合切开复位治疗跟骨骨折54例临床观察

Clinical Observation of 54 Cases of Calcaneal Fracture Treated with Internal and External Application of Chinese Herbal Medicine

目的:观察中药内服外敷联合切开复位治疗跟骨骨折的临床疗效。方法:采用临床随机对照试验,将54例跟骨骨折的患者随机分成对照组与研究组,每组27例。对照组患者术前、术后均采用患抬高患肢、早期冰敷、功能锻炼、静脉滴注甘露醇等治疗;研究组在对照组治疗基础上加用院内制剂消肿止痛方内服外敷。观察两组出现皮肤皱褶的时间、踝周径差值及足周径差值、术后出现皮肤愈合不良或坏死发生率、术后患肢疼痛情况。结果:与对照组相比,研究组在治疗前后出现皮肤皱褶的平均时间明显缩短、踝周径差值及足周径差值均明显增大、降低术后出现皮肤愈合不良或坏死发生率、减轻术后患肢疼痛,且差异均具有显著统计学意义(P<0.05)。结论:院内制剂消肿止痛方外敷内服缩短手术前出现皮肤皱褶的平均时间,减轻足踝部肿胀,缩短术前等待时间和术后住院时间,降低术后出现皮肤愈合不良或坏死发生率、减轻术后患肢疼痛,促进患者早期康复。

Objective： To observe the clinical efficacy of internal fixation and external application of Chinese herbal medicine in the treatment of calcaneal fracture. Methods： a total of 54 patients with calcaneal fractures were randomly divided into two groups： control group and study group, 27 cases in each group. Patients in the control group before and after surgery were used to raise the limb, early functional exercise of ice, and intravenous infusion of mannitol treat- ment; the study group in the control group based on the use of hospital preparations Xiaozhongzhitong internal and external party. The time of skin fold, the difference of ankle circumference and the difference of foot circumference between the two groups were observed. The incidence of postoperative skin healing or the incidence of necrosis and the pain of the postoperative limb were observed. Results： compared with control group, the study group before and after treatment appear wrinkled skin, shorten the average time of ankle circumference difference and foot circumference dif- ference were significantly increased and decreased postoperative healing or skin necrosis rate, reduce postoperative limb pain, and the differences were statistically significant （P〈0.05）. Conclusion： hospital preparation of Xiaozhongzhi- tong recipe to shorten the average operation time before oral skin wrinkles, relieve ankle swelling, shorten the waiting time of preoperative and postoperative hospitalization time, healing or skin necrosis, reduce the incidence of postopera- tive limb pain reduction after operation, promote early rehabilitation of patients.
