膝关节镜联合复位内固定治疗胫骨平台骨折的效果分析

Effect of arthroscopic combined with reduction and internal fixation fortibial plateau fractures

目的 研究分析膝关节镜联合复位内固定治疗胫骨平台骨折的效果.方法 入选病例数共有70 例,均为胫骨平台骨折患者,年龄、男女构成等资料无统计学意义,根据治疗法不同,随机将入组实验的胫骨平台骨折患者70 例分为两组,对照组35 例采取切开复位内固定术治疗,实验组35 例患者联合膝关节镜和复位内固定术治疗,对比二者手术时间、切口长度、下床活动时间、术后并发症发生、功能恢复情况以及骨折愈合时间.结果 实验组手术时间（82.32±27.38）min,切口长度（4.56±1.24）cm,下床活动时间（2.01±0.87）周,骨折愈合时间（3.27±0.45）个月,功能优良愈合率为91.4%,术后并发症发生率为8.6%;对照组手术时间（113. 22±32.71）min,切口长度（9.21±4.32）cm,下床活动时间（3.98±0.33）周,骨折愈合时间（5.69±0.79）个月,功能优良率为77.1%,术后并发症发生率为25.7%.通过分析,实验组手术时间、切口长度、功能优良率、下床活动时间、骨折愈合时间以及术后并发症发生率都优于对照组（P 〈 0. 05）.结论 联合膝关节镜和复位内固定术对胫骨平台骨折患者进行治疗,创伤小,效果显著,可促进患者尽早康复,价值高,值得推广应用.

Objective To i nvestigate t he effect of a rthroscopy c ombined w ith r eduction a nd i nternal f ixation for t ibialplateau fractures. Methods： 60 cases of patients with tibial plateau fracture, there is no statistically significant difference inage, composition of men and women. According to the treatment method, randomly divided into control group （N = 35） andexperimental group （N = 35）, the control group was treated with open reduction and internal fixation, the experimental groupwas treated with arthroscopic combined with reduction and internal fixation,the time of operation, length of incision, activitytime of bed, postoperative complication, function recovery and fracture healing time were compared. Results The experimentGroup：time of operation （82.32±27.38） min, incision length （4.56±1.24） cm, activity time （2.01 ± 0.87） weeks, fracture healingtime （3.27 ± 0.45） months, good healing rate of 91.4%, postoperative complication rate of 8.6%; the control group： time ofoperation （113.22±32.71） min,incision length （9.21±4.32） cm,activity time （3.98±0.33） weeks, fracture healing time （5.69±0.79）months, good healing rate of 77.1%, postoperative complication rate of 25.7%;the experimental group was better than thecontrol group in operation time, incision length, function good rate, bed activity time, fracture healing time and postoperativecomplication rate（P 〈 0.05）. Conclusion Arthroscopic combined with reduction and internal fixation for the treatment of tibialplateau fractures, small trauma, significant effect, can promote early recovery of patients, high value, worthy of application.
