神经内镜及显微镜下垂体瘤切除术的临床比较分析

Comparative Analysis of Clinical Microscopic and Endoscopic Resection of Pituitary Tumors

目的:比较经鼻蝶入路神经内镜与显微镜下垂体瘤切除术的临床疗效。方法:选择2010年8月~2013年8月笔者所在医院收治的垂体瘤患者146例,按照手术方式不同分为观察组(84例)和对照组(62例),分别采用神经内镜和显微镜下进行治疗,比较两组手术时间、住院时间及并发症发生率(鼻出血、蝶窦炎、脑脊液鼻漏、神经血管损伤等);随访6个月,观察手术全切率、激素恢复及视力视野的变化情况。结果:两组住院时间、手术时间比较,观察更优,差异有统计学意义(P<0.05)。结论 :与显微镜手术比较,经鼻蝶入路神经内镜切除垂体瘤具有简单快捷、损伤小、肿瘤全切率高、术后患者恢复快及并发症少等优点。

Objective： To compare the clinical efficacy between microscopic and endoscopic resection of pituitary tumors. Methods：146 patients with pituitary tumor from Jan 2013 to May 2015 were divided into observation group（84 cases） and control group（62 cases）according to the operation method,treated with endoscopic and microscopic resection respectively. The operation time,hospitalization time and the incidence of complications（ bleeding from the nose,sphenoidal sinusitis,cerebrospinal fluid rhinorrhea,nerve and blood vessel injury,etc.） of the two groups was compared. Followed up for 6 months,the total rate of the surgery,the recovery of the hormone and the changes of visual field were observed. Results： Compared with the control group,the operation time and hospitalization time of the observation group significantly were shorter,difference the was significant statistically（ P 0. 05）. Conclusion： Compared with the microscopic operation,endoscopic resection of pituitary tumor is simple,fast,low damage,high tumor total resection rate,postoperative recovery of patients with fast and less complications.
