膝关节镜清理术联合中医分期论治治疗膝关节痛风性关节炎86例临床分析

Clinical Analysis of Knee Arthroscopy and Staged TCM Differentiation and Treatment of 86 Cases with Knee Joint Gouty Arthritis

目的探讨关节镜下手术清理配合中药分期施治对治疗膝关节痛风性关节炎的临床价值。方法方便选取2012年6月—2016年6月期间该院收治的86例膝关节痛风性关节炎患者为研究对象,并将其分为观察组(n=43)和对照组(n=43),观察组患者行关节镜引导下的刨削清理,组织射频汽化去除关节滑膜,软骨面及半月板表面吸附的尿酸盐结晶、增生组织,同时处理剥脱的关节软骨及成形受侵蚀的半月板,术后按急性期、缓解期辩证选用中药等内科治疗。对照组患者仅接受膝关节镜下清理术治疗,对比观察两组术后满意度及Lysholm症状功能评分。结果观察组患者术后满意度(93.02%)明显高于对照组(76.74%),且观察组患者术后Lysholm症状功能评分(78.26±19.23)分远远优于对照组(60.46±16.82)分,两组比较差异有统计学意义(P<0.05)。结论关节镜下清理术配合中医分期论治能够明显缓解疼痛,改善关节功能,降低复发风险,值得临床推广。

Objective To study the clinical value of knee arthroscopy and staged TCM differentiation and treatment of 86 cases with knee joint gouty arthritis. Methods 86 cases of patients with knee joint gouty arthritis admitted and treated in our hospital from June 2012 to June 2016 were conveniently selected and divided into two groups with 43 cases in each, the observation group adopted the arthroscopic-guided gouging, and adopted the internal medicine, and the control group only adopted the knee arthroscopy and the satisfactory degree after surgery and Lysholm symptom function scores of the two groups were observed and compared. Results The postoperative satisfactory degree in the observation group was obviously higher than that in the control group(93.02% vs 76.74%), and the postoperative Lysholm symptom function score in the observation group was much better than that in the control group[(78.26±19.23)points vs(60.46±16.82)points], and the difference between the two groups was statistically significant(P<0.05). Conclusion The knee arthroscopy and staged TCM differentiation and treatment can obviously relive the pains, improve the joint function and reduce the reoccurrence risks, and it is worth clinical promotion.
