皮肤恶性肿瘤574例回顾性分析

Retrospective Analysis of 574 Patients with Skin Cancer

目的分析皮肤恶性肿瘤的临床资料,了解其临床特点及发病情况。方法回顾本院2004-2013年病理诊断为皮肤恶性肿瘤的病例资料,并进行数据分析。结果共入选574例皮肤恶性肿瘤,常见皮肤恶性肿瘤534例,其中基底细胞癌251例(47.00%)、鳞状细胞癌140例(26.22%)、Kaposi’s肉瘤81例(15.17%);男女性别无差异;病程超过一年者85.39%。40岁以下的基底细胞癌患者占13.15%(33/251);头颈部仍是基底细胞癌的好发部位(88.45%)。在临床诊断与病理诊断符合率方面,常见皮肤恶性肿瘤的符合率在70.00%左右,而皮肤附属器等少见皮肤恶性肿瘤的符合率仅为30.00%。结论在新疆地区常见的皮肤恶性肿瘤中,头面曝光部位仍是好发部位,最常见的皮肤恶性肿瘤是基底细胞癌,Kaposi’s肉瘤位居常见皮肤恶性肿瘤的第三位,临床上应重视中青年皮肤恶性肿瘤的发生,皮肤附属器恶性肿瘤临床少见易误诊,仍是临床诊断的难点。

Objective To investigate the clinical features and changes of the skin cancer patients. Methods Clinical data of skin cancer lesions confirmed histopathologically from 2004 to 2013 in our hospital were retrospectively analyzed by using statistical methods. Results A total of 574 patients with skin cancer, common cancer were 534 cases, basal celI carcinoma accounted for 251 patients （47. 00% ） , squamous cell carcinoma were 140 patients （26. 22% ）, and Kaposi＇ s sarcoma were 81 patients（ 15.17% ）. There was no difference between male and female. The duration of more than one year was 85.39%. Basal cell carcinoma patients under 40 years old was 13.15% （ 33/251 ）. The head and neck was the predilection site of basal cell carcinoma （88.45%）. The coincidence rate in clinical and pathological diagnosis of skin cancer was about 70.00% , skin appendages and other rare skin cancer was only 30. 00%. Conclusion Among the common skin cancer in Xinjiang area, head and face are still the predilection sites. Basal cell carcinoma is the most common skin cancer, while Kaposi＇s sarcoma is the third common skin cancer. Clinical attention should be paid to young skin cancer, and skin adnexal malignant tumor is rare and easily misdiagnosed. The clinical diagnosis is still difficult.
