彩色多普勒超声联合肿瘤标记物在胃部肿瘤定性诊断中的价值研究

The diagnostic value of color Doppler ultrasound combined with tumor markers on gastric tumor

目的:探讨彩色多普勒超声联合肿瘤标志物在胃部肿瘤定性诊断中的价值。方法:选取2016年1月至2017年5月在聊城市第二人民医院发现的胃部肿瘤患者65例,全部病例均经病理证实其良恶性。所有患者均行超声检查,分析其超声图像和血流特点,并对患者进行血清肿瘤标志物水平测定,然后根据超声影像表现联合肿瘤标志物对胃部肿瘤的良、恶性进行判断。结果:65例胃部肿瘤患者,恶性肿瘤35例,良性肿瘤30例。其中,彩色多普勒血流显示Ⅱ~Ⅲ级信号恶性病变组为25例(25/35),良性病变组为6例(6/30),具有统计学差异。超声通过影像表现及血流特点诊断恶性肿瘤26例,与病理诊断符合率为74.3%(26/35);血清肿瘤标记物诊断恶性肿瘤21例,与病理结果符合率60%(21/35);超声诊断联合肿瘤标记物与病理诊断符合率94.3%(33/35),差异具有统计学意义(P<0.05)。结论:彩色多普勒超声联合肿瘤标志物可以显著提高胃部肿瘤诊断的准确率,是鉴别胃部肿瘤良、恶性的有效方法。

Objective To explore the value of color Doppler ultrasound combined with tumor markers in the diagnosis of gastric tumor. Methods From lan 2016 to IUlY 2017, sixty-five gastric tumor patients were collected in the Second Peoples Hospital of Liaocheng. The ultrasound charateristics and the value of tumor markers were analyzed to diagnose benign and malignant gastric tumor. Results In 65 patients,there were 35 cases of patients with therioma and 30 cases of patients with benign tumor. The blood flow of patients with therioma in grade Ⅱ～Ⅲ were 25 cases（25/35）,while only 6 cases of benign tumor（6/30）,it had statistically significant difference（P 〈 0.05）. The diagnositic coincidences of ultrasound and tumor markers were 74.3% （26/35） and 60% （21/35） , while coincidences of ultrasound combined with tumor markers were 94.3% （33/35） , they had statistically significant difference（P 〈 0.05）. Conclusion ultrasound combined with tumor markers can be very useful in diagnosinz benign or malignant gastric tumox＇.
