伴有皮肤缺损的断指再植应用指侧方皮瓣和静脉皮瓣修复的疗效观察

Clinical Observation on Finger Lateral Skin Flap and Vein Flap in Replantation of Amputated Finger With Skin Defect

目的分析伴有皮肤缺损的断指再植应用指侧方皮瓣和静脉皮瓣修复的疗效。方法选取本院2015年1月—2017年3月收治的420例伴有皮肤破损的断指再植患者作为此次实验对象。随机将其分为对照组与观察组,每组各210例。对照组患者采用指侧方皮瓣修复治疗,观察组患者采用静脉皮瓣修复治疗,比较两组患者治疗效果及有无发生皮瓣坏死、感染等并发症情况。结果治疗后,观察组患者治疗良好率为98.10%;对照组为97.62%,组间对比,差异不具有统计学意义(χ~2=0.055,P=0.815);观察组中,4例(1.90%)发生并发症,对照组中,5例(2.38%)发生并发症,组间对比,差异不具有统计学意义(χ~2=0.055,P=0.815)。结论使用指侧方皮瓣修复术和静脉皮瓣修复术治疗伴有皮肤缺损的断指再植患者都能取得良好的疗效,无严重不良反应发生,在临床治疗时可以根据患者具体情况选择治疗方案。

Objective To analyze the efficacy of finger lateral skin flap and vein flap in replantation of amputated finger with skin defect. Methods 420 patients with severed fingers were treated in our hospital from January 2015 to March 2017. The patients were divided into two groups： control group and observation group, 210 cases in each group. Patients in the control group were treated with lateral skin flap, patients in observation group were treated with venous flap repair. The curative effect, complications of skin flap necrosis and infection were compared between the two groups. Results After treatment, the good rate of treatment in the observation group was 98.10%, and that in the control group was 97.62%. The difference between the two groups was not statistically significant （χ^2=0.055,χ^2=-0.815）; In the observation group, 4 cases （1.90%） had complications, and 5 cases （2.38%） had complications in the control group. The difference between the two groups was not statistically significant （~=0.055, P=0.815）. Conclusion The use of repair and repair of lateral skin flap venous flap treatment combined with skin defect of finger replantation patients can achieve good curative effect, no serious adverse reactions in clinical treatment, can choose the treatment according to the specific circumstances of patients.
