门诊不合理处方干预分析及改进措施的探讨

Intervention Analysis and Improvement Measures of Irrational Prescriptions in Outpatient Department

目的分析探讨我院门诊不合理处方情况,并及时进行干预,充分发挥临床药师的作用。方法选取我院门诊2016年5月—2017年5月开据的387例不合理门诊处方进行分析,分析探讨门诊处方的不合理情况并给予改进措施。结果不合理门诊处方主要科室分布为内科、外科、妇产科和简易门诊;不合理处方的体现主要表现为处方不规范,用法用量不合理、遴选药品不适宜、重复用药以及抗菌药物使用不合理等方面。结论门诊处方不合理用药情况严重,门诊在开据处方时,需充分发挥临床药师的作用,提高处方的质量,规范临床用药,减少用药失误率,以提高我院的医疗质量。

Objective To analyse and discuss the incidences of irrational outpatient prescription in our hospital, and allow the pharmacists to do their best to take interventions in time. Methods In our hospital outpatient service from May 2016 to May 2017, according to the 387 cases of unreasonable outpatient prescriptions were analyzed, analysis of outpatient prescriptions unreasonable situation and to give improvement measures. Results The irrational prescriptions were mainly made by the internal medicine, surgical department, obstetrics and gynecology, and the simple clinic. The prescriptions＇ irrational aspects were unacceptable prescriptions, unreasonable usage and dosage, inappropriate selection of the medicine, repeated medication, unreasonable use of antibiotics etc. Conclusion The conditions of irrational outpatient prescription are serious. When the outpatient department make prescription, the pharmacists should have the chance to do their best to improve the quality of the prescription, and to standardize the use of medicine and decrease the misuse of medicine, so as to our hospital＇s medical quality can be improved.
