盐酸氨溴索联合纤维支气管镜治疗老年重症肺炎疗效观察 被引量:7

Clinical Observation on the Effect of Ambroxol Combined With Fiberoptic Bronchoscopy in the Treatment of Severe Pneumonia in the Elderly

摘要 目的探讨大剂量盐酸氨溴索联合纤维支气管镜吸痰对老年重症肺炎患者血气指标改善及机械通气时间的影响。方法选取2015年10月—2016年10月我院收治的78例老年重症肺炎患者作为研究对象。按随机数字表法,将其分为观察组与对照组,每组各39例。对照组给予常规剂量盐酸氨溴索+纤维支气管镜吸痰治疗,观察组给予大剂量盐酸氨溴索+纤维支气管镜吸痰治疗。统计对比治疗前后两组血气指标改善状况及机械通气时间。结果治疗后,观察组患者的动脉血氧饱和度、氧合指数及动脉氧分压均高于对照组,机械通气时间短于对照组,差异具有统计学意义(P<0.05)。结论大剂量盐酸氨溴索联合纤维支气管镜吸痰治疗老年重症肺炎患者可有效改善血气指标,减少机械通气时间。 Objective To investigate the effects of high dose ambroxol combined with sputum aspiration by bronchoscope on improvement of blood gas index and mechanical ventilation time in elderly patients with severe 'pneumonia. Methods 78 elderly patients with severe pneumonia from October 2015 to October 2016 in our hospital were randomly divided into observation group and control group by according to the random number table method, 39 cases in each group. The control group were given routine dose of ambroxol hydrochloride and sputum aspiration by bronchoscope, and the observation group were treated with high dose of ambroxol combined with sputum aspiration by bronchoscope. The improvement of blood gas index and the time of mechanical ventilation were compared between the two groups. Results After treatment, the arterial oxygen saturation, oxygenation index and arterial oxygen partial pressure in the observation group were higher than those in the control group, and the mechanical ventilation time was shorter than that of the control group, the difference was statistically significant (P〈0.05).Conclusion Large dose of ambroxol combined with sputum aspiration by bronchoscopy can effectively improve blood gas index and reduce mechanical ventilation time in elderly patients with severe pneumonia.
