糖尿病足患者生活质量水平及其影响因素

Quality of life and Influencing Factors in Patients with Diabetic Foot Ulcer

目的调查长沙市60例糖尿病足患者生活质量水平,探讨其影响因素。方法 2014年5-11月,便利抽样法选取湖南省长沙市两所三级甲等医院内分泌科住院或伤口门诊的60例糖尿病足(diabetic foot ulcer,DFU)患者为研究对象。采用修订后的中文版糖尿病特异性生活质量测定量表(diabetes quality of life measure,A-DQOL)对其进行调查,并分析结果。结果在A-DQOL中,DFU患者满意程度得分为(31.63±8.46)分,影响程度得分为(43.90±12.57)分,忧虑程度Ⅰ(与社会、家庭或职业有关)得分为(13.20±5.34)分,忧虑程度Ⅱ(与疾病有关的忧虑程度)得分为(8.13±3.94)分。DFU患者在上述4个维度评分及总分均低于DM非足溃疡患者,差异均有统计学意义(均P<0.05)。不同DM病程、DFU病程、年龄、文化程度、家庭人均月收入、糖化血红蛋白(hemoglobin Alc,HbAlc)水平、高密度脂蛋白(high-density lipoprotein cholesterol,HDL-C)水平的DFU患者的生活质量评分差异均有统计学意义(均P<0.05)。家庭人均收入、年龄、文化程度、HbAlc水平是DFU患者生活质量的主要影响因素。结论糖尿病足患者生活质量普遍偏低,相关部门应该针对其生活质量的主要影响因素积极加强对其干预和指导。

Objective To investigate the quality of life and find its influencing factors in patients with dia- betic foot ulcer in Changsha.Methods By convenience sampling,60 patients with diabetic foot ulcer （DFU） were selected and investigated by using the revised Chinese diabetes quality of life measure （A-DQOL）, and the results were analyzed.Results Satisfaction scored（31.63±8.46） ,effect scored（43.90±12.57） ,anxiety-scored （related by society, family and profession） （13.20 ±5.34）, anxiety [I scored （related by diseases） （8.13 ±3.94）.All scores in four aspects and the total score of DFU patients were lower than diabetic patients without ulcers （all P〈0.05）.The statistical significance of quality of life score were found in DFU patients with different DM disease stage, DFU disease stage, age, educational background, average family income,HbAlc level and HDL-C level （all P 〈 0. 05 ）. Family income, age, educational background and HbAlc level were the influencing factors were DFU patient＇s quality of life.Conclusion Because of the low quality of life in patients with diabetic foot ulcer,intervention and guidance in improving the quality of life is critical for patients with diabetic foot ulcer.
