微创法与传统凿骨劈冠法拔除下颌低位近中阻生第三磨牙的临床效果对比

Minimally invasive surgery versus traditional chisel crown method in extraction of impacted mandibular third molar

目的对微创法与传统凿骨劈冠法拔除下颌低位近中阻生第三磨牙的效果进行分析和评价。方法选取2015年4月至2016年4月本院接收治疗的102例下颌低位近中阻生第三磨牙患者，依照随机数字表法分成探究组和常规组，各51例；常规组采取传统凿骨劈冠法治疗，探究组采取微创术治疗，对比分析两组拔牙时间、术中并发症情况，并应用改良版术后症状严重度量表（PoSSe）评测术后患者的生活质量。结果探究组的拔牙时间平均为（20．4±3．2）min，常规组为（33．7±4．1）min，两组比较差异有统计意义（P〈0．05）；探究组术中发症发生率、PoSSe各项评分均低于对比组，差异均有统计意义（均P〈0．05）。结论在下颌低位近中阻生第三磨牙拔除中，应用微创术的效果更确切，且手术时间更短，整体优于传统术式效果。

Objective To analyze and evaluate the effects of minimally invasive method and versus traditional chisel crown method in the extraction of impacted mandibular third molar. Methods 102 patients with impacted mandibular third molar treated at our clinic from April, 2015 to April, 2016 were selected as study objects and were divided into a study group and a routine group using random number table method, 51 cases for each group. The routine group took traditional chisel crown extraction and the study group received minimally invasive treatment. The postoperative extraction time and complications of the two groups were compared and analyzed. The patients＇ quality of life were evaluated by PoSSe. Results The average extraction time was （20.4±3.2） min in the study group and （33.7±4.1） min in the routine group, with a statistical difference （P〈0.05）. The incidence of intra-operative complications and the score of PoSSe were lower in the study group than in the routine group, with statistical differences （P 〈 0.05）. Conclusions Minimally invasive surgery in the extraction of impacted mandibular third molar is more effective and needs shorter operation time than traditional chisel crown method.
