清肺十八味丸联合桉柠蒎肠溶软胶囊治疗老年COPD合并肺结核病的疗效及其安全性观察

Clinical Efficacy of Qingfeishibaweiwan Combined Eucalyptol on Chronic Obstructive Pulmonary Disease with Pulmonary Tuberculosis in Elderly Patients

目的观察清肺十八味丸联合桉柠蒎治疗老年COPD合并PTB的临床疗效。方法将98例COPD合并PTB患者分为观察组及对照组,均给予抗结核治疗,对照组同时给予桉柠蒎,观察组在此基础上另给予清肺十八味丸。治疗后,观察两组综合疗效、病灶吸收、空洞闭合疗效及痰涂片转阴率。治疗前后,检测2组患者肺功能(FEV1、DLCO、FEV1/FVC),炎性因子(hs-CRP、TNF-α、IL-6、IFN-γ)水平。结果观察组综合疗效、病灶吸收、空洞闭合疗效总有效率、痰涂片转阴率分别为91.8%、95.9%、69.4%、67.3%,明显高于对照组(均P<0.05);治疗后,观察组肺功能明显高于对照组,炎性因子明显低于对照组(均P<0.05)。结论清肺十八味丸联合桉柠蒎肠溶软胶囊治疗老年COPD合并PTB临床疗效较好,值得深入研究。

Objective To explore the effect of Qingfeishibaweiwan combined eucalyptol on COPD with PTB in elderly patients. Methods 98 cases of COPD with PTB elderly patients are divided into control group （49 cases） and treatment group （49 cases）. The patients are given antituberculosis therapy. The control group is given eucalyptol, while the Qingfeishibaweiwan are given in the treatment group on the basis of control group. The general clinical effect, absorption effect, closure effect and sputum negative rate are observed. Before and after the treatment, the levels of FEVI, DLCO, FEV1/FVC, hs-CRP, TNF-et, IL-6 and IFN-y are detected in the two groups. Results The total effective rate, absorption effect, closure effect and sputum negative rate are in treatment group are 91.8%, 95.9%, 69.4% and 67.3%, significantly higher than control group （P 〈 0.05）. After the treatment, the levels of FEV1, DLCO, FEV1/FVC, hs-CRP, TNF-a, IL-6 and IFN-y are significantly promoted in treatment group than control group （P 〈 0.05）. Conclusion Qingfeishibaweiwan combined eucalyptol on COPD with PTB in elderly patients is affirmative and worth to the next study.
