胃癌组织中COX-2、VEGF-C表达及其与淋巴结转移的相关性的Meta分析

The Meta-analysis of the Expression of COX-2,VEGF-C in Gastric Carcinoma and Their Correlations with Lymphnode Metastasis

目的:探讨胃癌组织中环氧合酶-2(COX-2)和血管内皮生长因子-C(VEGF-C)的表达及其与胃癌淋巴结转移之间的关系。方法:利用计算机检索万方数据库、维普数据库、中国生物医学文献数据库和CNKI等数据库,手工检索所有纳入试验的参考文献,评价纳入研究的方法学质量,采用REv Man 5.3软件进行Meta分析。结果:Meta分析结果显示,胃癌组织的COX-2蛋白、VEGF-C蛋白表达阳性率分别为66.19%(513/778)、59.51%(463/778),明显高于正常胃组织的12.46%(39/313)、8.62%(27/313),差异有统计学意义(P<0.05)。研究显示淋巴结转移阳性组COX-2阳性、VEGF-C阳性的发生率为76.97%(391/508)、76.97%(370/508),明显高于淋巴结转移阴性组的44.81%(121/270)、36.30%(98/270),差异有统计学意义(P<0.05)。在胃癌组织COX-2阳性组中,VEGF-C表达率为74.46%(382/513),明显高于COX-2阴性组的64.91%(172/265),差异有统计学意义(P<0.05)。结论:COX-2、VEGF-C在胃癌组织中呈过表达,与胃癌的淋巴转移关系密切;COX-2、VEGF-C可能在促使胃癌微淋巴管形成过程中起作用。

Objective：To explore the expression of cyclooxygenase-2（COX-2） and vascular endothelial growth factor-C（VEGF-C） in gastric carcinoma tissue and theirs relationship with lymphnode metastasis.Method：Using the computer to search the related studies in Wanfang database,VIP database,Chinese Biomedical Literature Database and CNKI database,manually retrieve all the references included in the trial,methodological quality of the studies included.Using Rev Man 5.3 software to Meta-analysis.Result：Meta-analysis showed that positive rate of the expression of COX-2 protein,VEGF-C protein in gastric carcinoma,were 66.19%（513/778）,59.51%（463/778）,significantly higher than the normal stomach tissue of 12.46% （39/313）,8.62%（27/313）,both of them with gastric cancer were statistically significant（P〈0.05）.Studies had shown that the positive expression of COX-2 and VEGF-C in the positive group of lymphnode metastasis was 76.97%（391/508）,72.84%（370/508） respectively,which was significantly higher than those in the negative group of lymphnode metastasis,44.81%（121/270）,36.30% （98/270）（P〈0.05）.In COX-2 positive group of gastric carcinoma,the rate of VEGF-C expression was 74.46%（382/513）,and it was significantly higher than that of COX-2 negative group,which was 64.91%（172/265）（P〈0.05）.Conclusion： Over expression of COX-2 and VEGF-C are observed in gastric cancer,and closely related to lymphatic metastasis; and COX-2 and VEGF-C may play a role in promoting the development of gastric lymphoma.
