益气补肾活血通络法治疗糖尿病周围神经病变的系统评价

Systematic Evaluation of Yiqi Bushen Huoxue Tongluo Method in the Treatment of Diabetic Peripheral Neuropathy

目的运用Meta分析方法评价益气补肾、活血通络法治疗糖尿病周围神经病变(Diabetic peripheral neuropathy,DPN)的临床疗效。方法检索中国知网、中国生物医学期刊引文数据库、万方医学网、中国重要会议论文全文数据库、Cochrane Library、MEDLINE、ProQuest、PubMed等数据库中运用益气补肾活血通络法治疗糖尿病周围神经病变的随机对照临床试验文献,严格按照纳入标准筛选合格研究,应用stata12.0软件进行Meta分析统计相关数据,获得相对可靠的临床治疗证据。结果 7篇文献符合纳入标准,共643例患者,Meta分析结果显示:试验组患者的总有效率[OR=1.53,95%CI(1.20,1.95),Z=3.48,P<0.01],正中神经运动神经传导速度(motor nerve conduction velocity,MNCV)[MD=4.20,95%CI(3.77,4.63),P<0.0001],腓神经MNCV[MD=2.12,95%CI(1.86,2.83),P<0.0001],腓神经感觉神经传导速度(sensory nerves conduction velocity,SNCV)[MD=2.81,95%CI(2.48,3.14),P<0.0001]。结论益气补肾、活血通络法治疗糖尿病周围神经病变有一定的临床疗效,但目前研究质量偏低,总体研究例数较少,仍需大数据、多中心的随机双盲临床研究的进一步补充。

Objective：To evaluate the clinical effect of Yiqi Bushen Huoxue Tongluo method in the treatment of diabetic peripheral neuropathy（DPN） by metaanalysis. Methods：Randomized controlled clinical trial literature about diabetic peripheral neuropathy treated by Yiqi Bushen Huoxue Tongluo method was retrieved through CNKI, reference database of Journal of Chinese Biomedical Medicine, Wanfang Medical Science Network, conference database of Chinese Important Conference, Cochrane Library, MEDLINE, ProQues and PubMed, strictly in accordance with the inclusion criteria screening qualified research,and statal2. 0 software was used for metaanalysis of statistical data to access to relatively reliable clinical treatment evidence. Results：Seven papers were accorded with the criteria in which there were 643 patients. The Meta analysis showed the total effective rate of the patients in the trial group [ OR = 1.53,95% CI（1. 20 ,1. 95 ） ,Z =3.48,P 〈0.011 ,the median（MD =2. 20,95% CI（1.86,2. 83） ,P 〈0. 0001 ] ,peroneal nerve MNCV[ MD = 2. 12,95% CI （1.86,2. 83 ）, P 〈 0. 0001 ], and nervus peronaeus sensory nerves conduction velocity （SNCV） [ MD = 2. 81,95% CI （ 2. 48,3.14 ）, P 〈 0. 0001 ]. Conclusion ： Yiqi Bushen Huoxue Tongluo Method has certain clinical curative effect on the treatment of diabetic peripheral neuropathy. However, the current research quality is low and the number of the overall study cases is small. Large data, and multi - center randomized double - blind clinical study need further supplement.
