西藏自治区人民医院2016年麻醉药品和精神药品使用情况与合理性分析

Analysis of use of narcotic and psychotropic drugs and rationality of medication in People's Hospital of the Tibet Autonomous Region in 2016

目的:分析西藏自治区人民医院麻醉药品和精神药品的使用情况,为临床合理使用麻醉药品和精神药品提供依据。方法:通过HIS系统统计该院2016年麻醉药品和精神药品的使用数量和金额,以用药频度为指标,分析用药合理性。抽取该院2016年1―12月每月上、中、下旬每周一的麻醉药品和精神药品处方,采用EXCEL软件逐张录入处方的基本信息,并进行专项点评,分析不合理处方。结果:西藏自治区人民医院2016年使用麻醉药品12种,精神药品11种,共计23种。全年使用量和使用频次最高的麻醉药品和精神药品分别是枸橼酸芬太尼注射液和阿普唑仑片。处方点评共收集麻醉药品和精神药品处方1 642张,确定不合理处方共计373张,占比22.72%。主要不合理处方类型为开具麻醉药品和精神药品处方未执行国家相关规定(23.32%)、开具处方未写临床诊断或临床诊断书写不全(20.91%)、无适应证用药(15.55%)。结论:西藏自治区人民医院需进一步加强对临床医务人员的培训及对麻醉药品和精神药品的管理,以确保麻醉药品和精神药品临床使用的安全性和有效性,提高药物治疗水平。

Objective： To analyze the use of narcotic and psychotropic drugs in the People＇s Hospital of the Tibet Autonomous Region, so as to provide references for scientific management and rational use of these drugs in our hospital. Methods： The usage amounts of narcotic and psychotropic drugs in the People＇s Hospital of the Tibet Autonomous Region in 2016 were added up and the frequency of drug use （DDDs） was used as the index to evaluate the general situation of drug use. The prescriptions of narcotic and psychotropic drugs on every Monday of the first, second and last third of the month from January to December in 2016 were extracted, then Excel was employed to record the information of the patients, departments, and medications. The prescriptions were reviewed for rationality. Results： The hospital used 12 narcotics and 11 psychotropics in 2016. The top one narcotic and psychotropic drugs on the DDDs list were fentanyl for injection and alprazolam tablets, respectively. A total of 1 642 prescriptions were sampled, and irrational prescriptions accounted for 22.72% of the total. The main types of irrational prescriptions included non-compliance with the relevant national regulations （23.51%）, incomplete diagnosis （20.91%）, and lack of indications （15.55%）. Conclusion： The People＇s Hospital of the Tibet Autonomous Region needs to continuously strengthen the management of narcotic and psychotropic drugs and related training of the healthcare personnel, so as to ensure the safe and effective use of medications in clinic and improve the outcome of drug therapy.
