常规超声联合弹性成像及钼靶X线在乳腺微小肿块诊断中的应用价值

The Value of Conventional Ultrasound Combined With Elastic Imaging and Mammography in the Diagnosis of Small Breast Masses

目的研究常规超声联合弹性成像及钼靶X线在诊断乳腺微小肿块中的作用及价值。方法选择经手术病理确诊的乳腺微小肿块患者50例,分别行常规超声联合弹性成像、钼靶X线及三者联合检查,记录三种检查方式下乳腺肿块的良、恶性检出率、漏诊率及符合率,并进行对比分析。结果手术病理检查良性病灶29例,恶性病灶21例;常规超声联合弹性成像诊断乳腺微小肿块良性检出23例(79.31%),漏诊6例(20.69%),恶性检出13例(61.90%),漏诊8例(38.10%),总符合率72.00%;钼靶X线诊断良性检出22例(75.86%),漏诊7例(24.14%),恶性检出12例(57.14%),漏诊9例(42.86%),总符合率68.00%;三者联合检查诊断良性检出27例(93.10%),漏诊2例(6.90%),恶性检出20例(95.24%),漏诊1例(4.76%),总符合率94.00%。三者联合检查诊断检出率高,且与单独应用超声联合弹性成像或钼靶X均有统计学意义(P<0.05)。结论在乳腺微小肿块的筛选与检测中,常规超声联合弹性成像价值高于钼靶X线,且可以通过三者联合检查进一步提高乳腺微小肿块的准确率,值得临床借鉴。

Objective To evaluate the value of conventional ultrasound combined with elastic imaging and mammography X in the diagnosis of small breast masses.Methods Fifty patients with breast luminal masses diagnosed by operation and pathology were selected.Routine ultrasound combined with elastic imaging and mammography were examined by X-ray examination.The patho- logical results were compared with conventional ultrasound combined with elastic imaging, mammography and X Combined diagnosis of breast tumor mass, malignant detection rate, missed diagnosis rate and coincidence rate, and comparative analysis.Results Surgi- cal and pathological examination of benign lesions in 29 cases, 21 cases of malignant lesions;conventional ultrasound combined with elastic imaging in the diagnosis of benign breast mass examination in 23 cases （79.31% ）, missed diagnosis in 6 cases （20.69%）, malignant detection of 13 cases （61.90%）, missed 8 cases （38.10%）, the total compliance rate of 72.00% ;mammography X-ray di- agnosis, benign detection of 22 cases （75.86%）, missed diagnosis of 7 cases （24.14%）, malignant detection of 12 cases （57.14%）, missed 9 cases（42.86%）, the total compliance rate of 68.00% ;there were 27 cases （93.10%）, 2 cases （6.90%）, 20 cases （95.24%） of malignancy, 1 case（4.76%） of missed diagnosis, the total compliance rate was 94.00%.The combined detection of the three meth- ods was statistically signifieant（P〈0.05）,but the difference was statistically significant（P〈0.05）.Conclusion In the screening and detection of small breast masses, conventional ultrasound combined with elastic imaging value is higher than the molybdenum target X-ray ,the three joint examination can further improve the diagnosis of breast tumor mass accuracy, worthy of clinical reference.
