摘要
目的研究肥胖型和瘦型多囊卵巢综合征(PCOS)与酶类抗氧化剂和内分泌代谢指标的相关性。方法采用随机法选取该院2014-2016年确诊为肥胖型PCOS患者50例作为肥胖型组、瘦型PCOS患者50例作为瘦型组,并分别检测和分析两组的酶类抗氧化剂(包括SOD、CAT、GPX、INS)以及各种内分泌代谢指标和其相关性。结果瘦型组的PRL、E1、FSH、INS、SOD以及血脂4项均低于肥胖型组(P<0.05);瘦型组和肥胖型组的T、FSH均与SOD呈正相关关系,瘦型组的SOD与INS呈正相关关系,CAT与TC呈正相关关系;肥胖型组的CAT与SHBG、E1、E2、PRL、INS以及血脂4项呈正相关关系,SOD与SHBG、E1、E2、PRL、TC、TG、HOMA呈正相关关系。结论 PCOS患者本身会存在酶类抗氧化应激反应,且这种反应与PCOS患者拥有的无排卵现象以及高雄激素血症有着密切的联系,而且肥胖型PCOS的这种反应同时还会出现在内分泌代谢方面,但是瘦型PCOS的这种反应仅仅与胰岛素抵抗有密切的联系。
Objective To research the correlations between obese and lean type polycystic ovary syndrome (PCOS) and enzymatic antioxidants and endocrine metabolic indexes.Methods Fifty patients diagnosed as obese PCOS in the hospital from 2014 to 2016 were randomly selected as obese PCOS group,and 50 patients diagnosed as thin type PCOS in the hospital during the same period were randomly selected as thin type PCOS group.The enzymatic antioxidants [including superoxide dismutase (SOD),catalase (CAT),glutathione peroxidase (GPX),insulin (INS)] and endocrine metabolic indexes and the correlations in the two groups were detected and analyzed.Results Prolactin (PRL),estrone (E1),follicle-stimulating hormone (FSH),INS,SOD and blood lipids in thin type PCOS group were statistically significantly lower than those in obese PCOS group (P〈0.05);in the two groups,testosterone (T) and follicle-stimulating hormone (FSH) were positively correlated with SOD;in thin type PCOS group,SOD was positively correlated with INS,CAT was positively correlated with TC;in obese PCOS group,CAT was positively correlated with SHBG,E1,estradiol (E2),PRL,INS,and blood lipids,SOD was positively correlated with sex hormone binding globulin (SHBG),E1,E2,PRL,TC,TG,HOMA.Conclusion There is stress reaction of antioxidant enzymes in PCOS patients,and this reaction is closely correlated with ovulation and hyperandrogenism of PCOS patients.Stress reaction of antioxidant enzymes in obese PCOS patients also appears in endocrine and metabolic aspects,but in lean type PCOS patients,stress reaction of antioxidant enzymes is only correlated with insulin resistance.
