广西铁路系统女职工不良妊娠结局及危险因素

Adverse pregnancy outcomes and risk factors of female workers in Guangxi railway system

目的探讨铁路系统女职工不良妊娠结局及相关危险因素,为个性化制定职业女性孕期保健提供科学理论依据。方法对广西铁路系统1 511名女职工妊娠情况进行问卷调查,了解不良妊娠结局发生情况及影响女职工不良妊娠结局发生的危险因素。结果广西铁路系统女职工不良妊娠结局发生率为27.47%,前3位分别是低出生体重(19.19%)、巨大儿(5.82%)、难产(4.55%)。多因素二元logistics回归分析提示,工作岗位和孕期是否患妊娠并发症对女职工不良妊娠结局发生有影响。其他工作岗位女职工发生不良妊娠结局是担任列车员的女职工的0.852倍(OR=0.836),孕期患妊娠并发症的女职工发生不良妊娠结局是没有患孕期并发症的2.34倍(OR=2.342)。结论广西铁路系统女职工不良妊娠结局发生率高于一般妇女。担任列车员岗位和孕期患妊娠并发症的女职工是重点防治对象。相关铁路部门应积极采取措施,降低不良妊娠结局发生。

[Objective] To explore the adverse pregnancy outcomes and risk factors of female workers in railway system,provide a scientific basis for the individualized development of professional women＇s prenatal care. [Methods]A questionnaire survey was performed among 1 511 female workers in Guangxi railway system to understand the adverse pregnancy outcomes and related risk factors. [Results] The incidence of adverse pregnancy outcomes was 27.47% among female workers in Guangxi railway system,and the top three were the low birth weight（19.19%）,macrosomia（5.82%） and dystocia（4.55%）. Multivariate binary logistic regression analysis revealed that operating post and pregnancy complication were related to adverse pregnancy outcomes.The incidence of adverse pregnancy outcomes among female workers in other posts was 0.852 times as the female train attendants（OR=0.836）,and the incidence among female workers with pregnancy complications was 2.34 times as those without pregnancy complications（OR =2.342）. [Conclusion] The incidence of adverse pregnancy outcomes among female workers in Guangxi railway system is higher than that of common pregnancy women,and the prevention and control should focus on female train attendants and female workers with pregnancy complications. Relevant railway departments should take active measures to reduce the incidence of adverse pregnancy outcomes.
