围绝经期女性情绪障碍与综合干预的疗效分析

Analysis of mood disorders and comprehensive interventions in perimenopausal women

目的探讨围绝经期女性情绪障碍与激素补充治疗、心理疏导及抗焦虑干预的临床疗效。方法收集乌鲁木齐市第一人民医院、克拉玛依市人民医院和北京大学人民医院2015年10月至2016年10月妇科门诊围绝经期情绪障碍女性共100例,采用回顾性病例对照的方法,按接受治疗情况分为A、B两组,A组54例,仅给予绝经激素补充治疗;B组46例,在A组基础上给予抗焦虑药物干预;将同期乌鲁木齐市第一人民医院接受门诊体检的围绝经期健康女性50例纳入C组对照;比较三组女性情绪障碍程度,A、B两组用药前后的情绪评分变化及疗效比较。结果三组女性情绪障碍基础评分比较,A、B两组HAMA评分、HAMD评分、SAS评分、SDS评分比较差异均无统计学意义(P>0.05),分别与C组比较评分均明显增高(P<0.05);接受治疗后A、B两组四项评分均明显低于治疗前,两组治疗后各节点评分比较B组明显优于A组(P<0.05)。结论围绝经期女性情绪障碍通过激素补充治疗及抗焦虑综合干预后能有效缓解症状,提高生存质量,在临床上具有积极的实用价值。

Objective To investigate the mood disorders of perimenopausal women and the clinical effect of hormone replacement therapy, psychological counseling, and anti-anxiety intervention. Methods 100 cases of perimenopausal women with mood disorders were selected, which treated in the first people＇s hospital of urumqi, Karamay people＇s hospital or Peking university people＇s hospital form October 2015 to October 2016. They were divided into three groups according to the treatment. 54 cases as group A, only given postmenopausal hormone replacement therapy（HRT）. 46 cases as group B, given HRT and anti-anxiety drug interventions. 50 cases of postmenopausal healthy women as group C （control group）. The mood scores of the three groups were compared analysed. Results There was no statistic difference in HAMA scores, HAMD scores, SAS scores and SDS score （P〉0.05） between group A and group B. While compared with group C, scores of the former groups were significantly increased （P〈0.05）. After treatment, the scores of both group A and group B were significantly decreased, and group B was bette （P 〈 0.05）. Conclusion Hormone replacement therapy and anti-anxiety intervention could be effective in the treatment of mood disorders in perimenopausal women, and have a positive practical value in clinic.
